    Abstract: Mục đích: khảo sát sự phân bố của các loài nấm gây nhiễm khuẩn huyết, tiết niệu và tính kháng thuốc kháng nấm của chúng tại Bệnh viện Chợ Rẫy trong năm 2018. Vật liệu và Phương pháp: nghiên cứu hồi cứu. Tất cả các mẫu bệnh phẩm máu và nước tiểu cấy vi nấm gây bệnh dương tính trong năm 2018 tại khoa Vi Sinh bệnh viện Chợ Rẫy. Định danh vi nấm và thực hiện kháng nấm đồ bằng card YST và card AST YS07 trên hệ thống máy Vitek 2 Compact theo tiêu chuẩn CLSI. Kết quả: có 89 bệnh nhân cho kết quả cấy máu dương tính với nấm. Ba tác nhân hàng đầu gây nhiễm nấm máu lần lượt là C. tropicalis (31; 34,8%), C. albicans (20; 22,5%), C. glabrata (18; 20,2%). Tỉ lệ đề kháng của Candida spp. trong nhiễm nấm máu đối với Fluconazone là cao nhất (8; 9,9%), kế đến là Amphotericin B (7; 8,6%), Voriconazole (7; 8,6%), Flucytocine (1; 1,2%). Không thấy đề kháng với Caspofungin và Micafungin. Có 116 bệnh nhân cho kết quả cấy nước tiểu dương tính với nấm. Ba tác nhân hàng đầu gây nhiễm nấm nước tiểu lần lượt là C. tropicalis (43; 37,1%), C. albicans (41; 35,3%), C. glabrata (20; 17,2%). Tỉ lệ đề kháng của Candida spp. trong nhiễm nấm tiết niệu đối với Amphotericin B là cao nhất (8; 7,3%), kế đến là Fluconazole (5; 4,6%), Voriconazole (5; 4,6%), Caspofungin (3; 2,7%). Không thấy đề kháng với Flucytosine và Micafungin. Kết luận: Ba loại vi nấm thường gặp nhất trong nhiễm nấm máu và nước tiểu lần lượt là C. tropicalis, C. albicans và C. glabrata. Tỉ lệ đề kháng với thuốc kháng nấm của các chủng Candida spp. là dưới 10%. Trong nghiên cứu này, sự đề kháng của Candida spp. với thuốc kháng nấm là tương đối thấp, tuy nhiên cần theo dõi liên tục khuynh hướng kháng thuốc của các chủng nấm nhằm sớm phát hiện những thay đổi của tỷ lệ kháng thuốc để có hướng xử lý đúng đắn và kịp thời.

    Abstract: Acrobot, which is also called gymnastic robot, is a single input-multi output (SIMO) system which imitates motion of a barbell athlete. However, two links of this model are just equivalent to body and hand of athlete. To imitate better motion of athlete, three-linked acrobot- a complicated multi input-multi output (MIMO) under-actuated system – is developed. Parts of barbell athlete including leg, body and hand, are modelled well by this new system. In this paper, we present a method to apply an 81-rule fuzzy algorithm, which is popularly used for invert pendulum systems, to balance this model at upward equilibrium point. Instead of using a complicated 729-rule fuzzy controller which is set by knowledge of experts, we use more simple fuzzy structure. Through genetic algorithm (GA), the pre-processing and post-processing coefficients of fuzzy controller are chosen. With these suitable coefficients, our fuzzy controller is proved to work well through Matlab/Simulink simulation.

    Abstract: Constructed wetland is a low cost, effective technology and it is still in the state of improvement to enhance the treatment efficiency, especially in nutrient and trace elements treatment. This study investigated the effect of aerobic and anoxic conditions in Horizontal Subsurface Flow Constructed Wetland (HSFCW) on nutrient, organic, metal and Octylphenol - OP (Endocrine disrupting chemical) treatment. Two HSFCWs were constructed: HSFCW1 with three aerobic compartments; HSFCW2 with two aerobic compartments, one anoxic compartment. The two HSFCWs had the same design parameters (fiters, plants), except oxygen conditions. The results showed that aerobic and anoxic HSFCW may increase the efficiency of Nitrogen removal by 10%, but decreased by 11% in the efficiency of OP treatment (one of the EDCs). The efficiency of treatment of pollutants, including NH4+-N, COD, TP, Mn, Fe, Al and Cu between two HSFCWs were not significantly different; the average efficiency was 99%, 84%, 97%, 96%, 96%, 72% and 73%, respectively.     Abstract: Abstract Introduction: There are controversies over the treatment options for pelvic lymph node metastasis in low rectal cancer. The role of neoadjuvant radiotherapy in radical treatment of pelvic lymph node metastasis is still unidentified. Total mesorectal excision (TME) with Laparoscopic pelvic lymph node dissection (LPLND) provides lower pelvic recurrence in 5 years than TME only. Material and Methods: Prospective, uncontrolled clinical trial for patients with low rectal cancer (below peritoneal fold), who had suspected lateral pelvic lymph node metastasis on MRI scan and the patients did not have contraindication for laparoscopic surgery. Result: From January 2017 to February 2018 we performed 12 cases of LPLND. There was no con-version to open surgery. The average time for lymphadenectomy is 75 minutes with an average amount of blood loss of 97ml. Only 1 case had postoperative urinary retention (8%). The rate of pos-itive pelvic node was 5.8%, with an average of resected nodes of 4.9 nodes. Lymph nodes size on MRI scan in the positive pelvic node group was 20.6mm, compared to 7mm in the negative group. Conclusion: Laparoscopic lateral pelvic lymph node dissection is a feasible and safe technique and should be done by experienced colorectal surgeons. Aging, female sex and pelvic lymph node size on MRI are related to pelvic lymph node metastasis.

    Abstract: Abstract Introduction: Based on the obliterative suture technique or stapled transanal rectal resection procedure (STARR technique), Nguyen Trung Vinh introduced Suture Rectopexy Technique for Rectal Intussusception associated with Rectocele can not be treated by preservative management. This study aimed to to evaluate the recurrent factors after surgery. Patients and methods: 54 female patients with Obstructed Defecation Syndrome (ODS) underwent Suture Rectopexy for Rectal Intussusception associated with Rectocele from 2017 to 2020 at Trieu An Hospital. These patients were followed up and evaluated up to 18 months after surgery.The related factors evaluated were: Defecation Syndrome, ages, number of delivery, defecation duration, pre-operative ODS (Obstructive Defaecation Syndrome Score), size of rectocele on MRI, images of rectocele on MRI, post-operative urinary retention, anal stricture after surgery. Results: Suture Rectopexy Technique was performed on 54 patients, o f them, there were 40 patients had fully follow up for18 months. The improvement of defecation syndrome according to ROME IV after 18 months was 87.5%. ODS (Obstrucive defaecation syndrome Score) post surgery 18 months was 8,13 ± 3,95, the improvement was statiscally significant compares to pre surgery which was 14,6 ± 1,78 (P 〈 0,05). There were no severe complications recorded. The reccurent factor was found is the defecation duration before surgery. Conclusions: Suture Rectopexy Technique for Rectal Intussusception associated with Rectocele is highly efficient and safe. The reccurent factor was found is the defecation duration before surgery. This study was performed to evaluate the effectiveness and complications post-surgery of this method. Patients and methods: 54 female patients with Obstructed Defecation Syndrome (ODS) underwent Suture Rectopexy for Rectal Intussusception associated with Rectocele from 2017 to 2020 at Trieu An Hospital. These patients were followed up and evaluated up to 18 months after surgery. Results: Suture Rectopexy procedure to treat Rectal Intussusception associated with Rectocele was performed on 54 patients. Syndrome enhancement according to Guidelines-ROME IV standard post surgery 6 months was 94,4%, post surgery 12 months was 89,1%, post surgery 18 months was 87,5%. ODS score post surgery 6 months was 6,78 ± 3.08, 7,33 ± 3,54 post 12 months and 8,13 ± 3,95 post 18 months, the improvement was statiscally significant compares to pre surgery which was 14,6 ± 1,78. On post surgery fecal impaction MRI, Rectal Intussusception improved above 83,7% and Rectocele improved above 78% patients were evaluated. There were no severe complications. 77,7% patients after 6 months, 76% patients after 12 months and 77,5% patients after 18 months were satisfied with the results. Conclusion: Suture Rectopexy is highly efficient and safe in treating Intussusception combined with Rectocele disease. Tóm tắt Đặt vấn đề: Trên cơ sở nguyên tắc phẫu thuật của phẫu thuật khâu bít túi sa trực tràng hoặc cắt đoạn một khoanh thấp trực tràng (phẫu thuật STARR), tác giả Nguyễn Trung Vinh đưa ra phẫu thuật khâu treo để điều trị bệnh lý Sa niêm trong trực tràng (SNTTT) kết hợp Sa trực tràng kiểu túi (STTKT). Nghiên cứu tiến hành để đánh giá hiệu quả và biến chứng sau mổ của phương pháp. Đối tượng và phương pháp nghiên cứu: 54 người bệnh nữ có hội chứng tắc nghẽn đại tiện được phẫu thuật khâu treo điều trị sa niêm trong trực tràng kết hợp sa trực tràng kiểu túi từ 2017 đến 2020 tại bệnh viện Triều An. Các người bệnh này được theo dõi đánh giá đến 18 tháng sau mổ. Kết quả: Phẫu thuật khâu treo điều trị bệnh lý SNTTT kết hợp STTKT đã được tiến hành trên 54 người bệnh. Cải thiện triệu chứng theo tiêu chuẩn ROME IV sau mổ 6 tháng là 94,4%, sau mổ 12 tháng là 89,1%, sau mổ 18 tháng là 87,5%. Điểm ODS (Obstrucive defaecation syndrome) sau mổ 6 tháng là 6,78 ± 3.08, sau mổ 12 tháng là 7,33 ± 3,54, sau mổ 18 tháng là 8,13 ± 3,95, cải thiện có ý nghĩa thống kê so với trước mổ là 14,6 ± 1,78. Trên MRI động tống phân sau mổ, STTKT cải thiện trên 83,7% và SNTTT cải thiện trên 78% người bệnh được đánh giá. Không có biến chứng nặng nào đáng kể. 77,7% người bệnh sau 6 tháng, 76% người bệnh sau 12 tháng và 77,5% người bệnh sau 18 tháng hài lòng với kết quả phẫu thuật. Kết luận: Phẫu thuật khâu treo điều trị bệnh lý SNTTT kết hợp STTKT có hiệu quả cao và an toàn.     Abstract: Postherpetic neuralgia (PHN) is the most common and bearable complication of herpes zoster (HZ). This pain may have negative impact on the patient's all aspects of daily life and health‐related quality of life (HRQOL). Despite numerous advances in treatment, many patients remain resistant to the current therapy options. It is the first time subcutaneous injection of methylprednisolone acetate and lidocaine has been used to treat refractory PHN. We report the results of this treatment evaluating pain relief and HRQOL improvement in this disorder. Methods A total of 43 patients with refractory PHN was enrolled in the observational study. All patients received daily subcutaneous injection of methylprednisolone acetate and lidocaine for 10 consecutive days. The severity of pain was assessed by using Visual Analog Scale (VAS), and 36‐Item Short Form Survey (SF‐36) was applied to evaluate HRQOL. Assessment of the pain and HRQOL was carried out at baseline and posttreatment at 4 weeks as well as 6 and 12 months. Results At baseline, all patients experienced severe PHN with average VAS scores of 8.44 ± 0.85 (minimum 7; maximum 10). At 4 weeks, 6 months, and 12 months after treatment, the pain had significantly decreased ( P   〈  .001), and all subjects showed significant improvement in all eight domains of HRQOL. No major adverse events associated with the subcutaneous injection were observed. Conclusions Our results indicate that subcutaneous injection of methylprednisolone acetate and lidocaine can be an effective and safe treatment for PHN.

    Abstract: Law on Enterprises 2020 has appropriately modified provisions on the organization and operation of multi-member limited companies. However, shortly after implementation, some new regulations continuously lead to impediments whose legal loopholes would contribute to risks and harmfulness to corporations, investors, and even economic activities in general. Different from previous versions; the Law on Enterprises 2020 overlooks the rights and duties of companies’ members and supplies only provisions on those related to persons in the Board of Members instead. Neverthesless, this has forfeited several rights inevitably owned by a company’s member, who contributes to and owns parts of the company’s capital. In addition, it provides an incomplete regulation on corporations’ executive and management, in particular the apparent competence of the Board’s members. Currently, the Director (or General Director)’s decisions are scoped in corresponding with the rights of the Board of Members’ president. Regardless, the Corporation Law 2020 sends no confirmation of such rights. Accordingly, this paper extracts obstacles in rules on the Board’s members and their rights and duties for appropriate solutions. Under the common aspects, the problems raised on provisions concerning authorities to enter into contracts of the Director (or General Director) and the President of the Board of Members are considered for proper recommendations for further amendments to the Law on Enterprises.

    Abstract: Mục tiêu: Đặc điểm hình thái thùy trán trên phim MRI ở bệnh nhân nghiện rượu mạn tính. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả, cắt ngang ở 30 bệnh nhân nghiện rượu mạn tính điều trị nội trú tại Khoa Tâm thần, Bệnh viện Quân y 103 từ tháng 4/2021 đến tháng 3/2022 và 21 người nhóm chứng khỏe mạnh. Kết quả: Thể tích chất xám toàn bộ thùy trán ở nhóm bệnh giảm so với chóm chứng (91,83 ± 1,35cm3 so với 97,29 ±1,62cm3, với p 〈 0,05). Thể tích chất xám hồi ổ mắt bên phải và thể tích chất xám hồi trước trung tâm bên trái ở nhóm bệnh giảm so với nhóm chứng (5,96 ± 0,1 cm3 và 4,85 ± 0,11 cm3 so với 6,24 ± 0,13 cm3 và 5,20 ± 0,13 cm3, với p 〈 0,05). Độ dày chất xám hồi trán trên ở nhóm bệnh giảm so với nhóm chứng (2,55 ± 0,04 cm3 so với 2,68 ± 0,03 cm3, với p 〈 0,05). Thể tích chất trắng toàn bộ hồi trán trên, toàn bộ hồi trán giữa, toàn bộ hồi trán dưới và toàn bộ hồi trước trung tâm ở nhóm bệnh giảm so với nhóm chứng (34,27 ± 0,75 cm3; 37,16 ± 0,69 cm3; 14,01 ± 0,32 cm3 và 26,35 ± 0,48 cm3 so với 37,54 ± 0,90 cm3; 40,54 ± 0,83 cm3; 15,68 ± 0,38 cm3 và 30,08 ± 0,57 cm3, với p 〈 0,05). Kết luận: Thể tích chất xám toàn bộ thùy trán, thể tích chất trắng hồi trán trên, hồi trán giữa, hồi trán dưới và hồi trước trung tâm ở nhóm bệnh giảm so với nhóm chứng.
