Abstract:
Mục tiêu: Khảo sát tốc độ giảm độ lọc cầu thận ước tính (eGFR) ở người bệnh đái tháo đường (ĐTĐ) típ 2 ít nhất 5 năm.
Phương pháp: Nghiên cứu đoàn hệ quan sát trên 467 bệnh nhân ĐTĐ típ 2 tại bệnh viện quận 10 (thành phố Hồ Chí Minh). Có 439 BN được theo dõi từ 3 - 5 năm. Thử creatinin ít nhất hai lần trong năm và ước tính eGFR theo công thức CKD - EPI. Giảm eGFR 〈 1 ml/ph/1,73 m2 được xem là không tiến triển và giảm ≥ 4 ml/ph/1,73m2 là giảm nhanh eGFR.
Kết quả: Người bệnh tham gia có tuổi trung bình 61,7 và nữ nhiều hơn (67,5%). Có 35 người (7,5%) có eGFR 〈 60 ml/ph tại thời điểm thu dung. Trong thời gian theo dõi, 221 người (50,3%) có tốc độ giảm eGFR hàng năm lớn hơn 1ml/ph/1,73 m2 và 16% BN giảm nhanh eGFR. Tốc độ giảm eGFR trung bình hàng năm của toàn mẫu là -1,4 ml/ph/1,73 m2, không khác nhau ở hai giới hay theo các mức BMI, eGFR ban đầu và bệnh võng mạc ĐTĐ nhưng eGFR giảm nhanh ở người có albumin niệu lượng nhiều (-2,7 ml/ph/ năm so với 1,6 và 1,2 ml/ph, p = 0,04). eGFR tiến triển rất thay đổi trong 5 năm bất luận eGFR ban đầu, ngoại trừ người bệnh thận giai đoạn 4 có eGFR tiếp tục giảm nhanh.
Kết luận: Tốc độ giảm eGFR hàng năm giúp các bác sĩ dự đoán tốt tiến triển của bệnh thận mạn hơn trị số eGFR ban đầu. Nhận biết sớm người có giảm nhanh eGFR rất cần thiết cho việc đánh giá mức độ bệnh và theo dõi đáp ứng điều trị. Abstract
Aims: This study examined the decline rate of estimated glomerular filtration rate (eGFR) in patients who had type 2 diabetes (T2MD) for at least 5 years. Methods: A observational cohort study was conducted on 467 outpatients in a community - based hospital in Ho Chi Minh City. We followed 439 T2MD for 3 to 5 years. Serum creatinine was annually tested twice, on two occasions at least 3 months apart. The eGFR was estimated by the CKD-EPI equation. We defined subjects with an annual decline rate ≤ 1 ml/min/1.73 m2 as non - progression and a decline rate of at least 4 ml/min/1.73 m2 as rapid progression.
Results: Most patients were female (67.5%) with a mean age of 61.7 years. The prevalence of decreased eGFR was 7.5% (n = 35). During the follow - up period, 221 (50.3%) individuals had an annual decline rate greater than 1 ml/min/1.73 m2, and a rapid decline of eGFR was found in 16.0% of patients. The mean decline rate of eGFR was -1.4 ml/min/1.73 m2/year, and not different among sex, BMI, baseline eGFR, and diabetic retinopathy. The eGFR decline rate was faster in patients with severe albuminuria than in the normal or moderate albuminuria groups (-2.7 ml/min vs. 1.2 and 1.6 ml/min, p = 0.04). Progression of eGFR showed many changes irrespective of the baseline eGFR except that the G4 subgroup (baseline eGFR 〈 30 ml/min) continuously decreased.
Conclusions: An absolute annual eGFR decline rate can help clinicians predict the progression of chronic kidney disease better than eGFR levels. Early recognition of individuals with rapid loss of renal function is definitely important in chronic kidney disease care.
