Abstract:
Der primären Form des Hyperparathyreoidismus liegt meist ein Nebenschilddrüsenadenom mit erhöhter Parathyrin‐Ausschüttung zugrunde. Die Symptome dieses Krankheitsbildes ergeben sich vorrangig aus der Hypercalcämie und Hypophosphatämie. Auffinden und Operation des Adenoms erfordern vom Chirurgen ein hohes Maß an Geduld und Ausdauer, zumal die Adenome gelegentlich nicht an der Schilddrüse lokalisiert sind. Sie können sogar im Mediastinum placiert sein. Die Epithelkörperchenadenome wurden intraoperativ u. a. mit Methylenblaulösung (4 mg/kg KG) unter EKG‐Monitor‐Kontrolle dargestellt. Heute bedient man sich u. a. der Computer‐Tomographie. Vorteilhafter wäre ein aussagefähiges Scintigramm, wie es durch 125 J für die Schilddrüsenoperation erhalten wird. 99m Tc ist ein für die Nuklearmedizin optimaler Gammastrahler: die kurze Halbwertszeit von ca. 6,0 h gewährleistet eine geringe Strahlenbelastung, die Energie der Gammastrahlung entspricht mit 140 KeV dem Optimum der γ‐Kameras, das zwischen 100‐200 KeV liegt. Partikelstreuung tritt nicht auf. 99m Tc wird aus 99 Mo‐ 99m Tc‐Generatoren als Na‐Pertechnetat erhalten. Das Pertechnetat wird nur für die Scintigraphie der Schilddrüse genutzt, während andere Organe, z. B. Niere und Leber, über organische 99m Tc‐Komplexe abgebildet werden können. Zur Synthese dieser Komplexe muß das Pertechnetat im allgemeinen mit SnCl 2 reduziert werden. Die Liganden für die entstehenden Tc‐Oxo‐Kationen (z. B. TcO 2+ bzw. mit den Oxidationsstufen +5 oder +3) variieren für die verschiedenen Organe. So wird für die Knochenscintigraphie 2,3‐Dicarboxy‐1,1‐propan‐diphosphonsäure (DPD) eingesetzt. Obwohl der Nuklearmedizin schon zahlreiche 99m Tc‐Komplexe für verschiedene Indikationen zur Verfügung stehen, gibt es dennoch Organe, deren Darstellung wegen des Fehlens geeigneter Liganden für die Tc‐Komplexe z. Z. nicht möglich ist. So kann beispielsweise die Nebenschilddrüse noch nicht scintigraphiert werden.
