Abstract:
A szerzők a szabadgyök-reakciók dinamikáját vizsgálják a transzplantált betegekben. A szervtranszplantáció előtt álló betegek szervezetében a kóros metabolizmus következtében szabadgyök-felszaporodás figyelhető meg, ezért antioxidáns-kapacitásuk csökken. Amennyiben a beültetett szerv megfelelően működik, a homeostasis szabadgyök-antioxidáns egyensúlya helyreáll. A korai posztoperatív időszakban kialakuló szövődmények (akut rejectio, infekció, akut tubularis necrosis, cholestasis) következtében a szabadgyök-reakciók száma szaporodik. A szerzők részletezik, hogy melyek azok a szabadgyök-folyamatokban részt vevő mediátorok (interleukin-6, C-reaktív protein, szérumamiloid-A, neopterin) és enzimek (dipeptidil-aminopeptidáz, glutation-S-transzferáz, mieloperoxidáz), amelyek változását érdemes vizsgálni a perioperatív szakban, mert jelzi a szabad gyökök felszaporodását. E paraméterek korai posztoperatív monitorozása alkalmas az akut rejectio előjelzésére, az antirejectiós terápia effektivitásának nyomon követésére. A legtöbb, általunk vizsgált paraméter mennyisége szignifikánsan növekszik akut rejectióban és infekcióban a kontrollhoz képest. Az immunrendszer aktiválódását igazolják, de differenciáldiagnózis felállítására nem alkalmasak. Ezek közül kivétel az interleukin-6 szintje, mert infekcióban szignifikánsan nagyobb értéket mértünk, mint akut rejectióban. A veseszövet-károsodás mértékének meghatározását vizeletenzim-meghatározással a beültetendő graftfunkció előjelzésére használtuk. A reaktív oxigén intermedierek károsító hatását a kóros mennyiségben felszabaduló máj- és vizeletenzimek (dipeptidil-aminopeptidáz, glutation-S-transzferáz) is alátámasztják. Megfelelő tesztek kiválasztásával monitorozni lehet a transzplantált betegek műtét utáni szabad gyökös státuszát, illetve antioxidáns szerek adagolásával csökkenteni és ellenőrizni lehet a reaktív oxigén-intermedierek kontrollálatlan felszaporodását.
