    Abstract: 7502 Background: Chemoimmunotherapy is the standard first line of patients (pts) with diffuse large B-cell lymphoma (DLBCL) variant of Richter syndrome (RS). Response rate and duration are unsatisfactory. The biology of RS (high rate of DNA damage response pathway defects, high tumor mutation burden coupled with the expression of the PD1/PDL1 axis) prompts investigation of non-chemo combinations leveraging agents that circumvent TP53 abnormalities and trigger anti tumor immune response. MOLTO is a multicenter international ph 2 study (NCT04082897) evaluating activity and safety of atezolizumab (humanized monoclonal antibody blocking PD-L1), venetoclax (BCL2 inhibitor) and obinutuzumab (anti-CD20 MoAb) combination in untreated DLBCL-RS. Methods: Treatment consisted of 35 cycles with obinutuzumab (1000 mg C1-8), atezolizumab (1200 mg C1-18) and venetoclax (400 mg/d C1-35), q21. Primary endpoint was ORR ≥67% at C6. RS diagnosis was centrally revised. RS mutation profile was tested on pre-treatment cell free DNA. Minimal residual disease (MRD) was tested by 12 colors flow cytometry and NGS on peripheral blood mononuclear cells and plasma. Results: 28 planned pts were enrolled from Oct 2019 to Oct 2022 (Table). Three were not evaluable for primary endpoint due to G5 infection (n=1) or early withdrawn (n=2). As per intention-to-treat ORR was 67.9% (19/28) thus meeting primary endpoint. CR rate was 28.6%. No clinical characteristics influenced C6-ORR. After a median follow-up of 11.6 mo, 11/19 pts (57.9%) are in continuous remission (8 on active therapy, 2 received allogenic transplant, 1 discontinued due to MDS) among them, 6 for ≥24 mo. Of the remaining 8 pts, 7 progressed after a median of 14 cycles, 1 died from sepsis at C9 in remission. Median duration of response, PFS and OS were 11.7, 16.2 and 31.6 mo, respectively. Out of the 13 patients who progressed, 4 received a salvage therapy and are alive at a median follow-up of 24.3 mo. Bulky disease and ECOG PS 〉 1 affected PFS. Rate of unmeasurable MRD, impact of mutations and chronic lymphocytic leukemia-RS clonal relation on outcomes will be presented at the meeting. Overall 43 G3-4 adverse events (AE) were recorded in 17 pts (60.7%), mostly hematological (51.2%). Any grade immune-related AEs were reported in 6 pts (G3-4 in 2), none led to discontinuation. No tumor lysis syndrome observed. Infections ≥G3 occurred in 6 pts, including 2 G5. One pt developed MDS. Conclusions: Atezolizumab, obinutuzumab and venetoclax combination is active in pts with untreated DLBCL-RS. This regimen led to durable remissions, longer than 2 yrs in one third of responders.     Abstract: Adverse drug reactions (ADRs) reduce patients' quality of life, increase mortality and morbidity, and have a negative economic impact on healthcare systems. Nevertheless, the importance of ADR reporting is often underestimated. The project “FarmaREL” has been developed to monitor and evaluate ADRs in haematological patients and to increase pharmacovigilance culture among haematology specialists. In 13 haematology units, based in Lombardy, Italy, a dedicated specialist with the task of encouraging ADRs reporting and sensitizing healthcare professionals to pharmacovigilance has been assigned. The ADRs occurring in haematological patients were collected electronically and then analysed with multiple logistic regression. Between January 2009 and December 2011, 887 reports were collected. The number of ADRs was higher in older adults (528; 59%), in male (490; 55%), and in non‐Hodgkin lymphoma patients (343; 39%). Most reactions were severe (45% required or prolonged hospitalization), but in most cases, they were fully resolved at the time of reporting. According to Schumock and Thornton criteria, a percentage of ADRs as high as 7% was found to be preventable versus 2% according to reporter opinion. Patients' haematological diagnosis, not age or gender, resulted to be the variable that most influenced ADR, in particular severity and outcome. The employment of personnel specifically dedicated to pharmacovigilance is a successful strategy to improve the number and quality of ADR reports. “FarmaREL”, the first programme of active pharmacovigilance in oncohaematologic patients, significantly contributed to reach the WHO “Gold Standard” for pharmacovigilance in Lombardy, Italy.

    Abstract: Current treatment of chronic lymphocytic leukemia (CLL) patients often results in life-threatening immunosuppression. Furthermore, CLL is still an incurable disease due to the persistence of residual leukemic cells. These patients may therefore benefit from immunotherapy approaches aimed at immunoreconstitution and/or the elimination of residual disease following chemotherapy. For these purposes, we designed a simple GMP-compliant protocol for ex vivo expansion of normal T cells from CLL patients’ peripheral blood for adoptive therapy, using bispecific Ab blinatumomab (CD3 × CD19), acting both as T cell stimulator and CLL depletion agent, and human rIL-2. Starting from only 10 ml CLL peripheral blood, a mean 515 × 106 CD3+ T cells were expanded in 3 wk. The resulting blinatumomab-expanded T cells (BET) were polyclonal CD4+ and CD8+ and mostly effector and central memory cells. The Th1 subset was slightly prevalent over Th2, whereas Th17 and T regulatory cells were & lt;1%. CMV-specific clones were detected in equivalent proportion before and after expansion. Interestingly, BET cells had normalized expression of the synapse inhibitors CD272 and CD279 compared with starting T cells and were cytotoxic against CD19+ targets in presence of blinatumomab in vitro. In support of their functional capacity, we observed that BET, in combination with blinatumomab, had significant therapeutic activity in a systemic human diffuse large B lymphoma model in NOD-SCID mice. We propose BET as a therapeutic tool for immunoreconstitution of heavily immunosuppressed CLL patients and, in combination with bispecific Ab, as antitumor immunotherapy.

    Abstract: Introduction: Patients (pts) with CLL may be at particular risk of severe COVID-19 given advanced age and immune dysregulation. Two large series with limited follow-up have reported outcomes for pts with CLL and COVID-19 (Scarfò, et al. Leukemia 2020; Mato, et al. Blood 2020). To provide maximal clarity on outcomes for pts with CLL and COVID-19, we partnered in a worldwide effort to describe the clinical experience and validate predictors of survival, including potential treatment effects. Methods: This international collaboration represents a partnership between investigators at 141 centers. Data are presented in two cohorts. Cohort 1 (Co1) includes pts captured through efforts by European Research Initiative on CLL (ERIC), Italian CAMPUS CLL Program, and Grupo Español de Leucemia Linfática Crónica. The validation cohort, Cohort 2 (Co2), includes pts from US (66%), UK (23%), EU (7%), and other countries (4%). There is no overlap in cases between cohorts. CLL pts were included if COVID-19 was diagnosed by PCR detection of SARS-CoV-2 and they required inpatient hospitalization. Data were collected retrospectively 2/2020 - 5/2020 using standardized case report forms. Baseline characteristics, preexisting comorbidities (including cumulative illness rating scale (CIRS) score ≥6 vs. & lt;6), CLL treatment history, details regarding COVID-19 course, management, and therapy, and vital status were collected. The primary endpoint of this study was to estimate the case fatality rate (CFR), defined as the proportion of pts who died among all pts hospitalized with COVID-19. Chi-squared test was used to compare frequencies; univariable and multivariable analyses utilized Cox regression. Predictors of inferior OS in both Co1 and Co2 were included in multivariable analyses. Kaplan-Meier method was used to estimate overall survival (OS) from time of COVID-19 diagnosis (dx). Results: 411 hospitalized, COVID-19 positive CLL pts were analyzed (Co1 n=281, Co2 n=130). Table 1 describes baseline characteristics. At COVID-19 dx, median age was 72 in Co1 (range 37-94) and 68 in Co2 (range 41-98); 31% (Co1) and 45% (Co2) had CIRS ≥6. In Co1, 48% were treatment-naïve and 26% were receiving CLL-directed therapy at COVID-19 dx (66% BTKi ± anti-CD20, 19% Venetoclax ± anti-CD20, 9.6% chemo/chemoimmunotherapy (CIT), 1.4% PI3Ki, 4% other). In Co2, 36% were never treated and 49% were receiving CLL-directed therapy (65% BTKi ± anti-CD20, 19% Venetoclax ± anti-CD20, 9.4% multi-novel agent combinations, 1.6% CIT, 1.6% PI3Ki, 1.6% anti-CD20 monotherapy, 1.6% other). Most pts receiving CLL-directed therapy had it held at COVID-19 diagnosis (93% in Co1 and 81% in Co2). Frequency of most COVID-19 symptoms/laboratory abnormalities were similar in the two cohorts including fever (88% in both), lymphocytosis (ALC ≥30 x 109/L; 27% vs. 21%), and lymphocytopenia (ALC & lt; 1.0 x 109/L; 18% vs. 28%), while others varied between Co1 and Co2 (p & lt;0.0001), including cough (61% vs. 93%), dyspnea (60% vs. 84%), fatigue (13% vs. 77%). Median follow-up was 24 days (range 2-86) in Co1 and 17 days (1-43) in Co2. CFRs were similar in Co1 and Co2, 30% and 34% (p=0.45). 54% and 43% were discharged while 16% and 23% remained admitted at last follow-up in Co1 and Co2, respectively. The proportion of pts requiring supplemental oxygen was similar (89% vs. 92%) while rate of ICU admission was higher in Co2 (20% vs. 48%, p & lt;0.0001). Figure 1 depicts OS in each cohort. Univariable analyses demonstrated that age and CIRS ≥6 significantly predicted inferior OS in both cohorts, while only age remained an independent predictor of inferior OS in multivariable analyses (Table 2). Prior treatment for CLL (vs. observation) predicted inferior OS in Co1 but not Co2. Conclusions : In the largest cancer dx-specific cohort reported, pts with CLL hospitalized for COVID-19 had a CFR of 30-34%. Advanced patient age at COVID-19 diagnosis was an independent predictor of OS in two large cohorts. This CFR will serve as a benchmark for mortality for future outcomes studies, including therapeutic interventions for COVID-19 in this population. The effect of CLL treatment on OS was inconsistent across cohorts; COVID-19 may be severe regardless of treatment status. While there were no significant differences in distribution of current lines of therapy between cohorts, prior chemo exposure was more common in Co1 vs. Co2, which may account for difference in OS.     Abstract: Introduction: The management of CLL has been transformed in recent years by the introduction of a number of targeted agents including the BCL2 inhibitor VEN, and the Bruton's tyrosine kinase inhibitor IBR. In the US and EU, VEN is currently approved for patients who have received at least 1 prior therapy, whereas IBR has wider indications that include front-line therapy as well as R/R CLL. The mechanisms of action of VEN and IBR are complementary (Bose et al. F1000Research 2017); this has led to studies of combination therapy in patients with CLL (Wierda et al. J Clin Oncol Suppl. 2018). In addition, the documented activity of VEN in patients with IBR R/R disease (Jones et al. J Clin Oncol Suppl. 2016), together with current approvals, has resulted in VEN being suggested as a reasonable treatment choice for patients who discontinue IBR (Byrd et al. J Clin Oncol Suppl. 2018). However, the clinical benefit of IBR in patients pretreated with VEN is less clear. We present a post-hoc series, obtained after follow-up of 8 patients with R/R CLL who received IBR after fixed-duration VEN+rituximab (R) therapy in the MURANO randomized phase 3 study (NCT02005471) (Seymour et al. N Engl J Med 2018). Methods: In MURANO, 389 patients with R/R CLL were enrolled and treated with 6 cycles of VEN+R followed by VEN monotherapy once-daily for up to 2 years, or 6 cycles of bendamustine (B)+R. Progression-free survival (PFS), the primary study endpoint, was based on investigator assessment. The present case series focuses on VEN+R patients from MURANO who developed progressive disease (PD), and who subsequently received IBR. Eight patients in the VEN+R group with PD subsequently received IBR (Table 1). The number of lines of therapy prior to VEN+R ranged from 1 (n=7) to 4 (n=1; median 1). Prior to VEN+R therapy, 7 of the patients (87.5%) received fludarabine+cyclophosphamide+R (FCR); of these, 3 patients (42.8%) achieved a complete response (CR), 3 (42.8%) achieved a partial response (PR), and 1 patient (14.2%) had stable disease (SD). Of these 7 patients, 2 were considered refractory to FCR before VEN+R therapy. At baseline (before VEN+R), three of the patients (37.5%) had chromosome 17p-deletion, and two patients (25%) had baseline lymph nodes ≥5 cm- 〈 10 cm. Baseline neutrophil counts ranged from 0.38-4.37 × 109/L, platelets from 59-228 × 109/L, and lymphocytes from 0.14-388.6 × 109/L. Best response to VEN+R treatment included 3 CRs and 5 PRs. The baseline status (chromosome and blood cell counts) of patients before IBR was not available. Seven of the patients (87.5%) achieved a response to IBR (6 PRs, 1 CR); 1 patient had SD after 3 months of treatment. IBR treatment duration ranged from 3 to 34 months. At last available follow-up, 6 patients were still on treatment and 2 had stopped treatment due to PD; none of the patients had died. Four of the 8 patients had IBR dose interruption or modification due to neutropenia (n=2), clarithromycin treatment (n=1), or cutaneous nevus biopsy (n=1). Multiple skin abscesses were reported in 1 patient. One patient had atrial fibrillation, and 2 patients had arthralgia (1 had nearly resolved on follow-up), and there were no reports of major bleeding. Conclusions: In this series of patients with R/R CLL who received IBR following prior VEN+R in the MURANO study, IBR showed clinical activity and acceptable tolerability, with no new safety signals. Our results suggest that the use of IBR after relapse following VEN+R is a reasonable option in patients with CLL. More data will be collected from MURANO on patients progressing after VEN+R who are subsequently treated with IBR monotherapy.
