Abstract:
Anesteziolozi se suocavaju sa porastom broja bolesnika koji treba da se podvrgnu nekardijalnim hirurskim intervencijama, a imaju povecan rizik od perioperativnih kardijalnih komplikacija. Ovo istrazivanje je imalo za cilj da utvrdi vrstu i ucestalost pojedinih perioperativnih (intraoperativnih i postoperativnih) kardijalnih komplikacija i korelaciju izmedju vrste izvedene hirurske intervencije, zivotnog doba bolesnika, postojanja pridruzenih oboljenja i ucestalosti perioperativnih kardijalnih komplikacija. Prospektivno istrazivanje je obuhvatilo 100 bolesnika sa kardijalnim oboljenjima koji su bili podvrgnuti nekardijalnim hirurskim intervencijama (Grupa A 50 bolesnika podvrgnutih intraperitonealnim operacijama i Grupa B 50 bolesnika podvrgnutih operacijama tumora dojke i stitaste zlezde). Bolesnici su praceni tokom perioperativnog perioda i nakon hirurske intervencije do izlaska iz bolnice u cilju utvrdjivanja eventualnih kardijalnih komplikacija (hipertenzija, hipotenzija, poremecaji srcanog ritma, poremecaji provodjenja, ishemija miokarda). Kardijalne komplikacije su postojale kod 64% bolesnika. Najcesce kardijalne komplikacije bile su hipertenzija, poremecaji srcanog ritma i elektrokardiografski znaci ishemije miokarda. Od 100 bolesnika, jedan (1%) je imao akutni infarkt miokarda sa smrtnim ishodom u postoperativnom periodu. Na ucestalost kardijalnih komplikacija nezavisno i statisticki znacajno su uticali: vrsta hirurske intervencije, starost bolesnika, trajanje operacije i anestezije, patoloski preoperativni elektrokardiografski nalaz, patoloski preoperativni rendgenski nalaz srca i pluca, i dijabetes.
