    Abstract: Results of the analytical development of data of the official statistical reporting on morbidity on negotiability of children and teenagers of the city of Moscow during 2005-2013 have shown the preservation of steady negative trends in the rise of the general morbidity, incidences of respiratory and school-related diseases, including diseases of musculoskeletal system, myopia etc. According to the longitudinal study of the state of health and physical development of the Moscow school pupils from 1 to 11 classes (2005-2015) the gain of the prevalence rate of functional disorders and chronic diseases in the course of school ontogenesis has been shown. The leading violations of students’ health with a tendency to increase in the prevalence rate are: functional violations of the musculoskeletal system, blood circulatory system, organ of vision, neuro-psychiatric disorders, as well as chronic diseases of musculoskeletal system, digestive and vision organs. In all age and sex groups of schoolchildren there is a reliable increase in body length and weight, chest circumference, acceleration of biological maturation in comparison with peers of the last decades. Indices of the muscular force of the right hand in modern schoolchildren have decreased in comparison with the supervision of 1960s and 1980s. The necessary condition of health preservation of children and teenagers is the providing safe and optimum conditions of education, training and life activity. The assessment of the efficiency of sanitary and epidemiological surveillance of training conditions of children and teenagers has shown an insufficient informational content of the assessment of a sanitary state of educational organizations on groups of sanitary and epidemiologic wellbeing (SEW). Institutions with the reduced SEW level (2 groups) were proved to have a significantly higher risk of formation of diseases, including school caused diseases. It has been developed a modern classification of the assessment of conditions and modes of training of children at the school characterizing the risk degree to health of students of training conditions (optimum, admissible, potentially dangerous and dangerous). In addition new standard and methodical documents on carrying out sanitary-epidemiological expertise for the provision of sanitary and epidemiological welfare of students in secondary schools have been developed and proved.

    Abstract: Introduction. The state of health in students in schools is directly dependent on the state of health and psycho-emotional well-being in teachers. The purpose of the study: to substantiate the algorithm for assessing the provision of favourable conditions for the professional activities of schoolteachers based on an analysis of the lifestyle, work intensity and work regime, the state of health and psychological well-being in teachers. Materials and methods. The object of the study were teachers, tools for assessing the health promoting activities of schools. Surveys of lifestyle, work management, psychological status in teachers, medical examinations were conducted; analysis of tools for assessing the health promoting activities of schools. Results. Teachers are mostly women aged 30 to 59 years, with an average teaching experience of 23 years, have favourable family and living conditions. The optimal intensity of the teacher’s activity was found in 3.3%, acceptable – in 76.7%, tense of the 1st degree (harmful) – in 20.0%. 74.1% of teachers had a high level of occupatonal stress. Most teachers care little for their health. The leading behavioural risk factors for teachers are irrational unbalanced nutrition, irrational management of work and rest, low physical activity. Limitations. The use of a complex of research methods was one of the strengths of the study, which made it possible to identify potential factors affecting the health of teachers and develop an algorithm for assessing the provision of favourable conditions for the professional activities of schoolteachers. The program of an in-depth examination with the determination of the state of health and psychological status included only primary school teachers in Moscow, so the results of the study may vary depending on the place of residence (region) and personal characteristics, but can be generalized in relation to persons with similar background characteristics and conditions Conclusion. Modern professional risk factors for the health of teachers are determined. For the first time, indicators and an algorithm for assessing the provision of favourable conditions for the professional activity of schoolteachers are proposed.

    Abstract: Introduction. Against the background of socio-economic transformations and the digitalization of education, the health indicators of students are changing, which requires in-depth research. Aim of the study: based on the longitudinal observation of the health of schoolchildren, to identify the patterns of its formation for the scientific justification of improving the medical care of students. Material and methods. From the 1st to the 11a grade (2005-2015), the same students (199 boys and 227 girls) of 4 Moscow schools were examined annually with the performance of anthropometry, electrocardiography, functional tests, and questionnaire to identify complaints and anamnesis; specialist doctors carried out 25,298 examinations. In addition to the analysis of morbidity, a qualitative analysis of the course of pathological processes in students was carried out to determine positive and negative dynamics. Results. Over 11 years, the prevalence of functional disorders increased by 14.7%, chronic diseases by 52.8%, the frequency of visual disturbances, diseases of the musculoskeletal system, digestive organs, neurotic reactions increased; new forms of risk behaviour and information dependence appeared. An unfavourable period of development of pathological processes in schoolchildren is training in grades 9-11. Compared with the 1990 morbidity rates obtained by similar methods, there was an increase in the prevalence of functional disorders among adolescents by 2-3 times and chronic diseases by 20-70%. When comparing physical development with peers of the second half of the XX century, the observed schoolchildren increased the average values of body length and weight with a decrease in functional indicators. Distance learning during the first wave of COVID-19 in spring 2020 has increased the prevalence of computer vision, carpal tunnel syndromes, and neurotic reactions. Discussion. The main patterns of health formation in school ontogenesis, coinciding with the change of generations and the transition to the digitalization of education, are revealed. Conclusion. The study results allow predicting an increase in the prevalence of neuropsychiatric disorders, myopia, scoliosis, computer-visual and carpal tunnel syndromes. To preserve students’ health, to counteract COVID-19, a modern model of school health care is proposed.

    Abstract: The studies of personality characteristic of young adolescents caused by their genetic predispositions have become very essential in recent years. In our study we have investigated the association of dopamine (D2) and serotonin (2A) receptor genes with personality traits in 360 young adults. We have found the influence of the combination of DRD2/ANKK1 and HTR2A genes on the personality traits measured on Rule-Consciousness and Self-Reliance scales in girls. The significant statistical difference on Self-control scale was established for polymorphism TaqIA of gene DRD2/ANKK1 in boys. Additionally, the differences on Social Boldness scale were observed after stratification of genotypes DRD2/ANKK1 and HTR2A.

    Abstract: Introduction. The article is devoted to the 100-year period (1921-2021) of the development of medical professional orientation (MPO) and medical professional counseling (MPC) in Russia. The aim of MPO is a helping student to develop their professional self-determination, taking into account the state of their health and the possible adverse impact of occupational factors on the pathogenesis of existing health disorders. MPO is performed by a doctor in two forms. The first is to introduce adolescents and their parents to the medical problems of choosing a profession. The second - MPC - individual counselling of persons with health disorders and specific recommendations on occupations, places of study and rational employment. Medical professional counselling is a necessary element of medical support for schoolchildren. Over the past 100 years, the theoretical and methodological approaches to conducting MPO and MPC have changed. The purpose of this expert-analytical study is to determine the relevance, prominent trends, features and challenges of professional medical counselling in current socio-economic, scientific and technical conditions. Material and methods. scientific literature and methodological documents on the issues of occupational health of adolescents, MPO and MPC; 35-year experience of conducting MPC of adolescents in Yaroslavl (more than 3,500 schoolchildren), the experience of teaching the basics of MPC to students at the pediatric faculties of Sechenov University and Yaroslavl State Medical University, and experience of conducting career guidance work in the “Medical Sechenov Pre-University.” Results. The concept, principles, methods, and regulatory documents required for the MPO and MPC have been established to have undergone specific changes in connection with scientific and technological progress, new scientific data on the impact of occupational factors on the health of adolescents, changes in the structure of school morbidity, transformations in the system of vocational education and the reform of socio-economic conditions in Russia. Conclusions. Professional medical consultation remains relevant, methodological approaches require changes in connection with the digitalization of the economy, and the circle of consultants should include adolescents and young people.

    Abstract: Objectives - to reveal the health dynamics in schoolchildren in Samara, to identify the types of pathological conditions. Material and methods. We used data from school medical records of pupils in grades 1-4 (2070 children), clinical examination of adolescents aged 14 years (788 people), and data of dynamic monitoring of schoolchildren aged 15-17 years (1002 people). Statistical processing was carried out according to the standard procedure using a package of Windows applications (Excel, Word), Statistica. Results. We revealed the decrease in the number of healthy children from 1st to 4th grade from 27.2% to 8.8%, and an increased quantity of pupils with functional and chronic health conditions. The morbidity analysis of the 1st grade schoolchildren demonstrated the high ranks of nervous system diseases, mental and behavioral disorders (289.3% and 241.6%), the third place was taken by musculoskeletal system pathologies (200.0%), in the fourth - diseases of the respiratory system (148.8%), in the fifth - diseases of the eye (144.1%). In the 4th grade the leader was the diseases of the musculoskeletal system (548.5%), diseases of the eye (374.4%), diseases of the digestive system (167.4%). In adolescents the pathology of musculoskeletal system was most often registered (376.0%), vision impairment was the following (346.6%). Conclusion. In respect to the nosology and dynamics of health problems in schoolchildren it is necessary to identify the risk factors and to implement targeted prevention programs.

    Abstract: Introduction. Currently, there are not enough hygienic studies to identify causal relationships between health disorders and behavioural risk factors. The purpose of the study: to assess the influence of behavioural risk factors on students’ health outcomes. Materials and methods. The object of the study were one hundred nine students in grades 3-6. Questionnaires and comprehensive medical examinations were conducted. The research materials were statistically processed using non-parametric analysis methods, descriptive statistics and relative risk calculations. Results. Data were obtained on the leading behavioural risk factors of modern students (physical inactivity, lack of sleep, unhealthy diet, low school satisfaction, high aggressiveness in communication with peers. Functional deviations and chronic diseases of the musculoskeletal system, nervous system and mental sphere, eyes and digestive system were formed among students. There were identified three groups of behavioural risk factors: 1) use of digital devices; 2) social wellbeing in school; 3) regime of the day. Limitations. The study is limited to the study of the influence of only behavioral risk factors on the health of students, the time of observation, the use of self-questionnaires, which can lead to recall bias, affect the accuracy of data, with the possibility of overestimation or underestimation of data. Conclusion. Quantitative dependences of the development of the most common functional deviations and chronic diseases in students on risk behaviors in the children’s population have been established. This indicates the importance of high levels of prevalence of risky forms of behaviour and their underestimation in the formation of pathological conditions and the need for measures to correct behavior dangerous to the health of children and adolescents.

    Abstract: Earthworms are an important soil taxon as ecosystem engineers, providing a variety of crucial ecosystem functions and services. Little is known about their diversity and distribution at large spatial scales, despite the availability of considerable amounts of local-scale data. Earthworm diversity data, obtained from the primary literature or provided directly by authors, were collated with information on site locations, including coordinates, habitat cover, and soil properties. Datasets were required, at a minimum, to include abundance or biomass of earthworms at a site. Where possible, site-level species lists were included, as well as the abundance and biomass of individual species and ecological groups. This global dataset contains 10,840 sites, with 184 species, from 60 countries and all continents except Antarctica. The data were obtained from 182 published articles, published between 1973 and 2017, and 17 unpublished datasets. Amalgamating data into a single global database will assist researchers in investigating and answering a wide variety of pressing questions, for example, jointly assessing aboveground and belowground biodiversity distributions and drivers of biodiversity change.

    Abstract: Abstract 755 Background: Adoptive transfer of T cells genetically engineered to express affinity-enhanced T cell receptors (TcRs) specific for self-antigens is one approach to generate potent anti-tumor immunity, and this approach is being evaluated in a number of clinical trials. Although persistence of engineered T cells is positively correlated with response rates, a recurring observation to-date has been poor T cell persistence beyond the initial treatment window. We have recently initiated a Phase II clinical trial (NCT01352286) in patients with recurrent multiple myeloma (MM), to evaluate the safety and activity of autologous T cells genetically engineered to express an HLA-A201 restricted, affinity-enhanced TCR that targets an epitope shared by the NY-ESO-1 and LAGE-1 cancer testis antigens commonly expressed by MM. Clinical study details and outcomes are reported elsewhere in this meeting (Rapoport, A, Stadtmauer, E. et al.). Here we report the in vivo expansion, bioactivity, trafficking, durable persistence, and anti-tumor activity of TcR modified cells in this trial. Methods: Subjects received high-dose melphalan, infusion of autologous hematopoietic stem cells on day 0, and infusion of gene engineered cells (GEC) on day +2. Persistence of GEC was assessed by quantitative ABI-Taqman based PCR and a qualified assay that detects the lentivirus backbone sequence. Surface expression of the introduced affinity-enhanced TcR α/β receptor was detected by multiparametric flow cytometry and TcR-specific dextramer™reagents. NY-ESO-1 and LAGE-1 antigen expression in marrow samples was assessed by quantitative ABI-taqman based PCR and inventoried primer/probe sets. Results: As of August 2012, we have screened marrow from 46 patients for NY-ESO- and LAGE-1 expression. Of these, 29 (63%) and 17 (37%) expressed LAGE-1 and NY-ESO-1, respectively by PCR. LAGE-1 co-expression was observed in all cases with NY-ESO-1. To date 12 patients have been infused with GEC. Robust engraftment was observed, with peak levels of gene-marked cells in peripheral blood and marrow ranging from 1 × 102-1 × 103 cells/μL and peak levels detected within the first 21 days post infusion. By day +100 the absolute number of GEC showed patient-specific and variable contraction, with a subset of patients maintaining GEC at 1 × 102-1 × 103 cells/μL, and a second subset where GEC contracted to 1 × 101-1 × 102cells/μL. Trafficking to and persistence of GEC in marrow was observed. At both early (up to day +30) and, in a subset of patients, late timepoints (post day+42), circulating CD3+ T cells could be detected with surface expression of the affinity enhanced TcR. Engineered cells are detected in all patients at their latest timepoint by Q-PCR. To date we see no evidence for TCR mispairing with endogenous TCR. Tumor-antigen and tumor-specific targeting by GEC was evaluated using Q-PCR and quantifying transcript levels of NY-ESO-1, LAGE-1, and CD138 (as a plasma cell marker) in marrow biopsies at enrollment, and days +42, +100, and +180. In the initial cohort of 5 patients, selective reappearance of CD138 transcript is observed in 4 cases (2 sCR, 1 VGPR, 1 PR), with non-detectable or very weak expression of LAGE-1/NY-ESO-1 transcripts, suggesting specific targeting of tumor cells that express LAGE-1/NY-ESO-1. In the fifth patient, who has progressive disease and low engineered cell persistence, CD138 and LAGE-1/NY-ESO-1 transcripts are both present above pre-treatment levels. A second infusion was recently given to this patient. Summary: Our studies show for the first time that adoptive transfer of affinity-enhanced TCR engineered T cells in MM patients results in prolonged T cell persistence, homing to marrow, and anti-tumor activity.
