Abstract:
Die Autoren stellen die Hypothese auf, dass insbesondere asphyktische, reanimationspflichtige reife Neugeborene nach der Geburt von einer Spätabnabelung (nach mehreren Minuten) profitieren
könnten. Dafür sprechen plausible pathophysiologische Zusammenhänge. Die Autoren haben für ihr Review die verfügbaren Studien zusammengetragen. Aufgrund der vorliegenden Daten erscheint es nachteilig, ein asphyktisches Neugeborenes (z. B. nach Schulterdystokie) rasch abzunabeln, obwohl dies in den aktuellen Leitlinien gerade in dieser Situation empfohlen wird. Durch
Kompression der Nabelschnur oder des Thorax ist der venöse Rückfluss stärker betroffen als der arterielle Fluss in die Plazenta. Sauerstoffreiches venöses Blut steht so nicht für die Versorgung des Fetus zur Verfügung. Das Neugeborene leidet bei der Geburt oftmals nicht nur unter einer Hypoxämie und konsekutiven Hypoxie, sondern womöglich häufig auch oder vor allem unter
einer Hypovolämie. Ein sofortiges Abnabeln, insbesondere wenn diese noch vor Beginn der Atmung stattfindet, könnte die Lage des ohnehin kompromittierten Neugeborenen noch weiter verschärfen. Verzögertes Abnabeln hingegen führt zu einer fetoplazentaren Transfusion sauerstoffreichen venösen Bluts, das eine bestehende Azidose puffern könnte. Zudem wird auf diese Weise das
Blutvolumen um bis zu 20% gesteigert, was zu einem Anstieg aller Blutbestandteile führt, z. B. rote und weiße Blutzellen, Thrombozyten, mesenchymale Stammzellen, Immunglobuline, Eisen. Außerdem kommt es zu einer Erhöhung der pulmonalen Perfusion, was zusätzlich zu einem Ausgleich einer bestehenden Hypoxämie oder Hypoxie beitragen könnte. Wird zu früh, vor Eröffnung der
Lungenstrombahn, abgenabelt, fehlt dem linken Ventrikel die für einen funtionierenden Kreislauf notwendige Vorlast. Sowohl tierexperimentelle als auch klinische Studien untermauern diese Auffassung. Die Autoren werfen die Frage auf, ob eine Reanimation kompromittierter Neugeborener bei intakter Nabelschnur günstiger sein könnte. Geburtshilfliche und neonatologische Teams
sollten noch enger zusammenarbeiten, um eine Verbesserung des neo natologischen Outcomes zu erreichen.
