Abstract:
Objetivos: La OMS recomienda las formulaciones de supositorios de artemisinina como tratamiento antes de la transferencia para niños que no son capaces de tomar medicación oral y no pueden llegar rápidamente a un centro para recibir tratamiento parenteral. Hemos investigado los factores que influencian la adherencia de los cuidadores con respecto a la recomendación de trasferencia después de que sus niños recibieran tratamiento con supositorios rectales de artesunato. Métodos: El estudio estaba anidado dentro de un estudio de intervención que involucraba el tratamiento previo a la derivación de todos los niños que venían a un dispensario comunitario porque no eran capaces de tomar tratamiento oral debido a vómitos repetitivos, letargia, convulsiones o consciencia alterada. Todos los pacientes que no cumplían con la recomendación de transferencia fueron estratificados por acciones tomadas tras recibir la recomendación: llevar a sus niños a una farmacia, a un curandero, o no buscar más tratamiento; y fueron añadidos a una selección aleatoria de pacientes que cumplieron con la recomendación de derivación. Los cuidadores de niños que fueron entrevistados sobre su estatus socioeconómico, conocimientos de malaria, recomendación de transferencia recibida y acciones tomadas después del tratamiento anterior a la derivación. Los datos de las entrevistas de 587 cuidadores fueron pareados con los síntomas de los niños, la hora del tratamiento, la llegada al centro sanitario u otras acciones tomadas tras el tratamiento pre‐derivación. Resultados: La mayoría de los cuidadores (93.5%) reportaron haber recibido la recomendación de trasferencia del dispensador comunitario de medicamentos. El riesgo (odds) de adherencia con respecto a esta recomendación fue tres veces mayor para niños con consciencia alterada y/o convulsiones, que para niños con otros síntomas [OR 3.47, 95% IC 2.32‐5.17, p 〈 0.001]. Cuando se les preguntaba, los cuidadores que recordaban cuando [OR 2.19, 95% IC 1.48‐3.23, p 〈 0.001] y porqué [OR 1.77, 95% CI 1.07‐2.95, p = 0.026] habían sido aconsejados de acudir a un centro sanitario – tenían mayor probabilidad de seguir el consejo de derivación. El coste no influenciaba la adherencia, excepto dentro del área de influencia de los centros que cobraban por sus servicios. En estas áreas, los costes dificultaban la adherencia en una proporción de 4 a 5 veces entre aquellos que habían pagado previamente por servicios de laboratorio [OR = 0.25; 95% IC: 0.09‐0.67, p = 0.006] o por consulta [OR 0.20, 95% IC: 0.06‐0.61, p = 0.005] comparado con aquellos que no lo habían hecho. Conclusión: Cuando reciben una recomendación de transferencia, los cuidadores de pacientes con síntomas graves se siguen más fácilmente el consejo que otros cuidadores. El cobro de los servicios sanitarios influye negativamente en la adherencia.
