    Abstract: The diagnosis of intestinal microsporidiosis has traditionally depended on direct visualization of the parasite in stool specimens or intestinal biopsy samples by light and/or electron microscopy. Limited information about the specificity and sensitivity of PCR for the detection microsporidia in clinical stool specimens is available. To establish a sensitive and specific method for the detection of microsporidia in clinical samples, we studied clinical stool specimens of 104 randomly selected human immunodeficiency virus-infected patients with diarrhea to compare light microscopy and PCR. Fluorochrome Uvitex 2B staining was used for light microscopy. To raise the sensitivity of PCR, we used a powerful and fast DNA extraction method including stool sedimentation, glass bead disruption, and proteinase K and chitinase digestion. PCR was performed with primer pairs V1-PMP2, V1-EB450, and V1-SI500, and the nature of the PCR products was confirmed by Southern blot hybridization. Microsporidiosis was diagnosed by light microscopy in eight patients. Ten patients tested positive for microsporidiosis by PCR. Enterocytozoon bieneusi was found in seven cases, and Encephalitozoon intestinalis was found in four cases. In one case a double infection with E. bieneusi and E. intestinalis was diagnosed by PCR, whereas light microscopy showed only E. bieneusi infection. PCR testing of stool specimens is useful for diagnosis and species differentiation of intestinal microsporidiosis in HIV patients.

    Abstract: Zusammenfassung. Fragestellung: Kinder und Jugendliche entwickeln nach interpersonellen Typ-2-Traumata häufig Symptome, die über die Posttraumatische Belastungsstörung (PTBS) hinausreichen. Zur Umschreibung der Symptomatik wurde die bisher wenig untersuchte Entwicklungstraumastörung (ETS; van der Kolk et al., 2009) vorgeschlagen. Methodik: Die Arztbriefe von n = 161 1- bis 18-jährigen Patienten (61 % weiblich) einer Spezialsprechstunde für traumatisierte Kinder und Jugendliche einer Kinder- und Jugendpsychiatrie wurden anhand eines strukturierten Codierungsbogens geratet und über einen adaptierten ETS-Algorithmus analysiert. Ergebnisse: 77 % der PatientInnen erlebten interpersonelle Typ-2-Traumata; 6 % erfüllten die adaptierte ETS-Diagnose. Alle ETS-Kriterien lagen bei Kindern und Jugendlichen mit interpersonellen Typ-2-Traumata häufiger vor als bei PatientInnen mit akzidentiellem bzw. Typ-1-Trauma, wobei die Unterschiede für die ETS-Kriterien B (Affektive und physiologische Dysregulation) und G (Teilhabebeeinträchtigungen) statistische Signifikanz auf dem angepassten Signifikanzniveau von 0.2 % erreichten. Signifikante Alters- oder Geschlechtsunterschiede wurden nicht gefunden. Die Gruppe der Kinder unter 7 Jahren wurde hinsichtlich ihrer posttraumatischen Symptomatik deskriptiv analysiert. Schlussfolgerungen: Die Ergebnisse zeigen, dass zwar viele Kinder und Jugendliche über die PTBS hinausreichende Symptome entwickelt haben, aber nur ein geringer Teil die ETS-Diagnose erfüllt hat. Angesichts der teilweise unspezifischen und widersprüchlichen Befunde erscheinen weitere Studien mit größeren Stichproben, den vollständigen ETS-Kriterien und diagnosespezifischen Instrumenten zu der bisher wenig empirisch erforschten ETS als sinnvoll und notwendig.

    Abstract: Cross-culturally effective, low-threshold therapies for refugees that can be carried out quickly are urgently required. Worldwide, therapies are lacking, particularly for preschool refugee children, which support coping and prevent chronification of posttraumatic stress. This pilot study examined eye movement desensitization and reprocessing (EMDR)-based group treatment for preschool refugee children in German daycare centers. Ten refugee preschool children aged 4–6 years ( n = 5 female) with posttraumatic stress disorder (PTSD) symptoms took part in an EMDR-based group treatment (with 2–4 children per group). PTSD symptoms were rated by parents and preschool teachers using items from the Child Behavior Checklist (CBCL 1½–5), pre- and posttreatment, and at 3-month follow-up. After treatment children tended to display less fear of animals or situations than before treatment. According to preschool teachers' perspective, the total number of PTSD symptoms dropped significantly at posttreatment ( d = .93) and at follow-up ( d = .81). Before the intervention, preschool teachers rated the children as being more defiant than their parents did ( p = .020). After the intervention, the PTSD symptoms of boys were significantly higher than those of girls ( p = .036) according to preschool teachers. The results indicate that timely psychotherapeutic interventions can be conducted with refugee children displaying PTSD symptoms in daycare centers. The efficacy of this intervention needs to be studied in a larger sample under controlled randomized conditions.
