Abstract:
Hintergrund: Die transkranielle Gleichstromstimulation (tDCS) wird seit 10 Jahren zur Behandlung der Depression beforscht. Eine Reihe doppelblinder und offener Studien konnte überwiegend eine positive Wirkung der tDCS als Zusatzbehandlung zur medikamentösen Therapie nachweisen. Jedoch gibt es keine größeren Studien zur Anwendung der tDCS in naturalistischem Patientengut ohne studienbedingte Vorselektion durch Ein- und Ausschlusskriterien. Methodik: Das vorgestellte Positionspapier bewertet die Erkenntnisse einer Kombinationsbehandlung mit tDCS und Antidepressivum und empfiehlt eine standardisierte klinische Anwendung. Ein Fallbericht einer standardisierten Anwendung zeigte die Möglichkeit einer Anwendung in der klinischen Routine. Resultate: Die Anwendung von tDCS und begleitender antidepressiver Therapie ist in einer Reihe von Studien untersucht worden, wobei sich Stromstärke und Dauer der Anwendung in den letzten Jahren immer weiter erhöhten. In dem berichteten Fall kam es unter Medikation mit Citalopram 20 mg und intensivierter tDCS (20 Behandlungen zu je 30 min) zu einer raschen und nachhaltigen Verbesserung der depressiven Symptomatik, auch wenn eine Remission knapp nicht erreicht wurde. Schlussfolgerung: Die tDCS scheint bei Verwendung intensivierter Stimulationsprotokolle sicher anwendbar und könnte möglicherweise eine schnellere und nachhaltigere antidepressive Wirkung als eine medikamentöse Monotherapie bewirken. Daraus könnte sich eine weitreichende Bedeutung für die Behandlungsdauern der Erkrankung ergeben.
