Abstract:
Essa revisão define as situações de elevação aguda da pressão arterial, caracterizadas como crise hipertensiva e pseudocrise hipertensiva. Crise hipertensiva é definida quando a pressão arterial se apresenta com valores maiores ou iguais a 180 x 120 mm Hg e é subdividida em emergência e urgência hipertensiva. O quadro clínico da emergência hipertensiva difere da urgência hipertensiva por apresentar risco iminente de morte devido à lesão aguda ou em desenvolvimento em órgãos-alvo, principalmente coração, cérebro, rins e artérias, fato que não ocorre com a urgência hipertensiva. Por outro lado, na pseudocrise hipertensiva, independentemente do valor da pressão arterial, não há evidências de lesão aguda em órgãos-alvo nem risco imediato de morte, com base na história clínica, no exame físico e nos exames complementares básicos realizados durante a avaliação do paciente. A discussão sobre mitos, verdades e condutas é baseada em casos clínicos sucintos que abordam a apresentação clínica das situações descritas abaixo.
