Abstract:
Thalassemia là bệnh di truyền lặn phổ biến tại Việt Nam. Xác định sớm người lành mang gen bệnh thalassemia, tư vấn trước sinh giúp hạn chế sinh con mắc thalassemia thể trung bình - nặng. Nghiên cứu mô tả cắt ngang với mục tiêu xác định tỉ lệ người lành mang gen và mô tả các biến thể gây bệnh trên đồng thời hai gen HBA, HBB dựa trên hồi cứu kết quả xét nghiệm gen thalassemia sử dụng phương pháp giải trình tự thế hệ mới (Next Generation Sequencing-NGS) và gap-PCR của 764 đối tượng đến kiểm tra trước hôn nhân, trước mang thai và trước sinh tại Bệnh viện Đại học Y Hà Nội từ 2/2023 đến 10/2023. Tỉ lệ người lành mang gen là 18,1%, trong đó mang gen HBA, HBB, cả HBA và HBB lần lượt là 11,8%, 5,0% và 1,3%. Các biến thể được phát hiện trên gen HBA (n = 100) gồm: SEA (70,0%), α3.7 (20,0%), còn lại gồm SEA + α3.7, α4.2, HbCS và THAI. Các biến thể phát hiện trên gen HBB (n = 48) là CD26 (50%), CD41/42 (27,1%), CD17 (16,7%), -28A 〉 G (4,2%), CD71/72 (2,1%). NGS và gap-PCR có thể giúp tăng tỷ lệ phát hiện các biến thể một alen của gen HBA, cũng như hạn chế bỏ sót các trường hợp mang đồng thời cả gen alpha và beta thalassemia.
