    Abstract: Ulcerative colitis (UC) represents chronic inflammation of the large intestine. Immune response plays an important role in disease genesis and progression. Activated leukocytes secrete several cytokines that actively regulate the inflammatory response in UC. The aim of this study was to determine levels of cytokines IL-17, IL-27, IFN-γ and TGF-β in patients with UC and to test them as biomarkers for disease. The blood samples of 24 patients with ulcerative colitis without previous treatment and 37 healthy individuals were analyzed. Serum levels of IL-17, IL-27, IFN-γ and TGF-β were measured using sensitive enzyme-linked immunosorbent assay (ELISA) kits. Serum levels of IL-17, IL-27, IFN-γ and TGF-β were increased in patients with UC, compared to healthy controls (p=0.022; p=0.001; p=0.001; and p=0.002; respectively). Ratios of cytokines IL-27/IL-17, IFN-γ/TGF-β and IL-17/TGF-β were significantly higher in group of patients with UC (p=0.002; p=0.002; p=0.003; respectively). Serum value of TGF-β higher than 20 pg/ml presents a highly sensitive and specific marker for UC. We believe that increased production and predominance of immunosupressive TGF-β may represent compensatory mechanism for ongoing pro-inflammatory processes in UC.

    Abstract: We first present the results of an experiment in which the passive properties of the urinary bladder were investigated using strips of rabbit bladder. Under the assumption that the urinary bladder had orthopaedic characteristics, the strips were taken in the longitudinal and in the circumferential directions. The material was subjected to uniaxial tension, and stress-stretch curves were generated for various rates of deformation. We found that the rates did not have a significantly effect on the passive response of the material. Additionally, the stress-stretch dependence during relaxation of the material when exposed to isometric conditions was determined experimentally. Next, we measured nonlinear stress-stretch dependence to determine the coefficients for this dependence in analytical form using a standard fitting procedure. The same approach was used to obtain the coefficients for the relaxation curves from the experimental data. Two constitutive laws, the nonlinear model for passive response and the creep model, were introduced within the shell finite element for geometrically and materially nonlinear analysis. We provide descriptions of the numerical procedures that were performed by considering the urinary bladder as a thin-walled shell structure subjected to pressure loading. The developed numerical algorithm for the incremental-iterative solution was implemented into the finite element program, PAK. The response of the urinary bladder was calculated for continuous filling, and the numerical and experimental results were compared through cystometrograms (pressure-volume relationships). We also present comparisons of the shapes and volumes of the urinary bladder obtained numerically and experimentally. Finally, the numerical results of the creep response, when placed under constant internal pressure, are provided for various stages of deformation.

    Abstract: Rene Favaloro je jedan od najznacajnijih hirurga 20. veka. Prvi je objavio u medicinskoj literaturi tehniku izvodjenja aortokoronarnog premoscavanja, kojom je pocela nova era u lecenju koronarne bolesti. Radeci na Cleveland klinici, zajedno sa Masonom Sonesom i Efflerom, nakon velikog broja operacija na otvorenom srcu i koronarografija, bio je deo tima koji ce prvi uraditi aortokoronarno premoscavanje, a nakon toga i koronarografski dokazati funkcionalnost anastomoze. Pet godina nakon njegove smrti, u vreme sve cesce upotrebe perkutane implantacije stentova, aortokoronarno premoscavanje je i dalje, za pojedine grupe pacijenata, metoda izbora lecenja koronarne bolesti. U vremenu kada i u nasoj zemlji pocinje da radi sluzba hitnih perkutanih intervencija u akutnom infarktu miokarda, o cemu je Rene Favaloro sa Masonom Sonesom diskutovao i zagovarao agresivno lecenje infarkta u njegovoj ranoj fazi, nagovestivsi potrebu razvijanja slicne metode, secamo se zivotnih i profesionalnih uspeha kardiohirurga koji je promenio dotadasnji tok koronarne bolesti. .

    Abstract: Uvod/Cilj. Upotreba Parsonnet skora u proceni postoperativnog trenda mortaliteta bolesnika nakon hirurske revaskularizacije miokarda vec je standardno dobro poznat model predikcije koji je nasao svoje mesto u svim svetskim kardiohirurskim centrima. Cilj naseg rada bio je da se proceni njihov uticaj na kvalitet zivota bolesnika pet godina nakon hirurske revaskularizacije miokarda, kao i da se ispita prediktivna vrednost skora kvaliteta zivota u poredjenju sa vec siroko koriscenim Paronnet skorom predikcije mortaliteta. Metode. Studija je obuhvatila 449 bolesnika, kod kojih je u toku 1999. i 2000. godine izvrsena hirurska revaskularizacija miokarda. Izuzeti su bolesnici kod kojih je istovremeno izvrsena i operacija zaliska, bolesnici sa komorbiditetima ili bez kompletne revaskularizacije miokarda. Sezdeset meseci posle operacije izvrsena je analiza kvaliteta zivota koriscenjem upitnika SF-36. Analizirano je sedam aspekata kvaliteta zivota, kao i ehokardiografski parametri i njihova korelacija sa aspektima kvaliteta zivota. Rezultati. Petogodisnje prezivljavanje iznosilo je 84,2%. Prosecni enddijastolni dijametar (EDD) je 55,3 ? 5,6 mm, endsistolni dijametar (ESD) 38,7 ? 5,6 mm, ejekciona frakcija (EF) 51,7 ? 9,6%. Dilatacija leve pretkomore, leve komore, ejekciona frakcija, multisegmentni ispadi u kontraktilnosti i umereno teska mitralna regurgitacija bili su u negativnoj korelaciji sa svim aspektima kvaliteta zivota (p 〈 0,001). Kriticne eokardiografske vrednosti za optimalni kvalitet zivota bili su EDD 〉 54,5 mm (senzitivnost 57%, specificnost 70%, RR = 1,386), ESD 〉 37,5 mm (senzitivnost 65%, specificnost 57% RR = 0,855), a EF 〈 50% ima najjacu prediktivnu vrednost (senzitivnost 61%, specificnost 70%, RR = 0,916). Zakljucak. Predperativni ehokardiogarfski parametri imaju vrlo visoku prediktivnu vrednost ne samo za postoperativno prezivljavanje vec i za kvalitet zivota bolesnika posle hirurske revaskularizacije miokarda.

    Abstract: Patients with EuroSCORE 〈 2 are usually considered to have a low surgical risk and the lowest mortality. In our study preoperative factors in a group of 250 consecutive low-risk patients (EuroSCORE 〈 2), who underwent first isolated coronary artery by-pass surgery during 1999 and 2000., were analyzed. Cumulative follow-up period was 1178.48 patient-years and the primary clinical outcome was all-cause mortality. Patients’ average age was 59.2±7.5 yr. The following preoperative risk factors of increased 5-year mortality were identified: older age (P 〈 0.001), smoking, prior non-recent myocardial infarction and reinfarction, anteroseptal localization of myocardial infarction (P 〈 0.001), poor ejection fraction 〈 =35% (P 〈 0.001), dilatative cardiomyopathy (P 〈 0.001), wall motion systolic index 〉2 (P 〈 0.001), left atrial dilatation (P 〈 0.001), mitral regurgitation more than 2+ (P 〈 0.001), presence of left main disease, triple vessel coronary artery disease (P 〈 0.001), absence of collaterals (P 〈 0.001) and presence of more than 3 distal anastomoses. Through the present study it has been shown that it is possible to identify a subgroup of patients with low operative mortality and excellent 5-year survival after surgical treatment for coronary artery bypass surgery using preoperative clinical, echocardiographic, coronarographic and intraoperative data, even in difficult conditions of the civil war in the region.

    Abstract: Respiratory rehabilitation leads to reduction of symptoms, strengthens extremity musculature and improves emotional state and management of daily activities in patients with respiratory diseases. Aim of our study was to determine quality of life and functionality of COVID 19 patients before and after respiratory rehabilitation program. The study was conducted at the Clinical Center, Kragujevac, from June to July 2020. The study was a prospective clinical trial and included 62 patients with the acutephase of COVID-19. Respiratory rehabilitation program started at hospital and continued at home for three months overall. Quality of life was measured by the EQ-5D-5L and patient’s functionality by The FIM score. All five dimension of EQ-5D-5L were higher after respiratory rehabilitation program as well as EQ-5D index score and VAS score (0.8516±0.202 and 53.31±17.129 before rehabilitation, 0.9147±0.074 and 64.53±8.368 after rehabilitation). Respiratory exercise showed significantly improvement in FIM total score from 104.48±12.880 to 106.21±9.791, as well as in FIM motor and cognitive subscores. Respiratory rehabilitation program improves quality of life and functionality of COVID 19 patients.

    Abstract: Background and Objectives: Ulcerative colitis is chronic and/or progressive inflammation of the colorectal mucosa and submucosa and represents one of two major inflammatory bowel diseases. Ulcerative colitis has been associated with increased risk of arteriosus and venous thrombosis. There are numerous factors responsible for this; one of them is platelet activation and aggregation. The objective of our study was to determine if different treatment options for ulcerative colitis have an impact on platelet aggregation. Materials and Methods: This research was a prospective, observational study and included 94 newly diagnosed patients with UC divided into four treatment groups. For all patients, we measured platelet aggregability by using an impedance aggregometry method with a multiplate analyzer before and after treatment with infliximab, adalimumab, vedolizumab and azathioprine. A Paired Samples t test was performed in order to determine the difference in platelet aggregability before and after a certain therapy, since the data followed a normal distribution. Taking into account the impact of some clinical characteristics, multiple linear regression was conducted for the purpose of estimating the effect of therapy on the level of reduction in platelet aggregability. Results: All four drugs significantly reduced platelet aggregability. After we excluded the influence of clinical and endoscopic scores and disease localization on the results, we found that infliximab had the greatest anti-platelet activity. Conclusions: In addition to the well-known traditional risk factors for atherosclerosis, activation and aggregation of platelets play a significant role in the development of arterial thrombosis, and our results suggested that therapy use for the treatment of UC, especially infliximab, can have a great impact on cardiovascular morbidity and mortality by decreasing platelet aggregability.
