    Abstract: Rhegmatogenous retinal detachment (RD) is a threatening visual condition and a human disease model for retinal degenerations. Despite successful reattachment surgery, vision does not fully recover, due to subretinal fluid accumulation and subsequent photoreceptor cell death, through mechanisms that recapitulate those of retinal degenerative diseases. Hydrophilic bile acids are neuroprotective in animal models, but whether they can be used orally for retinal diseases is unknown. Ursodeoxycholic acid (UDCA) being approved for clinical use (e.g., in cholestasis), we have evaluated the ocular bioavailability of oral UDCA, administered to patients before RD surgery. The level of UDCA in ocular media correlated with the extent of blood retinal barrier disruption, evaluated by the extent of detachment and the albumin concentration in subretinal fluid. UDCA, at levels measured in ocular media, protected photoreceptors from apoptosis and necrosis in rat retinal explants, an ex vivo model of RD. The subretinal fluid from UDCA-treated patients, collected during surgery, significantly protected rat retinal explants from cell death, when compared to subretinal fluid from control patients. Pan-transcriptomic analysis of the retina showed that UDCA upregulated anti-apoptotic, anti-oxidant, and anti-inflammatory genes. Oral UDCA is a potential neuroprotective adjuvant therapy in RD and other retinal degenerative diseases and should be further evaluated in a clinical trial.

    Abstract: To determine the evolution of fundus autofluorescence ( FAF ) patterns in chronic central serous chorioretinopathy ( CSCR ) over time. Methods We retrospectively studied the changes in FAF patterns over time in 157 eyes of 112 patients with chronic CSCR using the Heidelberg Retina Angiography with a 488‐nm excitation light and a 500‐nm cutoff barrier filter. Results The mean duration of follow‐up was 37.2 months. The most common baseline pattern was that of granular hypoautofluorescence (51.0%). The earliest change in chronic CSCR is diffuse hyperautofluorescence and it occurs approximately 4 months after the reported first episode. The most common change observed at this stage is a change within areas of hyperautofluorescence where hyper‐reflective dots appeared or disappeared. Change in FAF patterns from areas of hyperautofluorescence to hypoautofluorescence was slow. Only 25% of eyes showed such a change in pattern by 36 months. It takes an average of 24 months for granular hypoautofluorescent pattern to develop confluent hypoautofluorescence. There were no predictive patterns for the development of confluent CSCR . Conclusion Fundus autofluorescence ( FAF ) changes in CSCR evolve very gradually and so is not a good outcome measure for clinical trials.

    Abstract: Purpose . To report the long-term follow-up results in patients with cataract and primary open-angle glaucoma (POAG) randomly assigned to cataract surgery combined with micro-bypass stent implantation or phacoemulsification alone. Methods . 36 subjects with cataract and POAG were randomized in a 1 : 2 ratio to either iStent implantation and cataract surgery (combined group) or cataract surgery alone (control group). 24 subjects agreed to be evaluated again 48 months after surgery. Patients returned one month later for unmedicated washout assessment. Results . At the long-term follow-up visit we reported a mean IOP of 15,9 ± 2,3 mmHg in the iStent group and 17 ± 2,5 mmHg in the control group ( p = N S ). After washout, a 14,2% between group difference in favour of the combined group was statistically significant ( p = 0,02 ) for mean IOP reduction. A significant reduction in the mean number of medications was observed in both groups compared to baseline values ( p = 0,005 in the combined group and p = 0,01 in the control group). Conclusion . Patients in the combined group maintained low IOP levels after long-term follow-up. Cataract surgery alone showed a loss of efficacy in controlling IOP over time. Both treatments reduced the number of ocular hypotensive medications prescribed. This trial is registered with: NCT00847158 .

    Abstract: The aim of this study was to evaluate the effect of anti-vascular endothelial growth factor (VEGF) therapy on choroidal neovascularisation (CNV) complicating central serous chorioretinopathy (CSC) using multimodal imaging, and to identify possible predictive factors of the treatment response. Design Retrospective study. Methods Data of 27 eyes with CNV complicating CSC treated with anti-VEGF therapy (either ranibizumab or aflibercept) were reviewed. Response to anti-VEGF treatment was evaluated by change in visual acuity, intra/subretinal fluid modifications and CNV changes on optical coherence tomography angiography (OCTA). Univariate and multivariate analyses were performed to identify predictive factors for central retinal thickness (CRT) change and for the relative degree of treatment response (complete, incomplete or absent fluid reduction). Results CRT was significantly reduced at 32±15 days after 2.8±1.3 injections (p=0.0004) as was the subretinal fluid (p=0002). Complete fluid resorption was observed in 45% of cases. Best corrected visual acuity did not significantly improve (p=0.18). CNV area (p=0.09) and CNV flow area (p=0.07) did not significantly decrease. No changes in CNV pattern were noted. Univariate analysis identified greater CRT at baseline (p 〈 0.0001), greater amount of subretinal fluid (p 〈 0.0001), a shorter period of retinal fluid (p=0.04) and female gender (p=0.04) as predictors for CRT reduction. After multivariate analysis the factor of greater CRT at baseline (p 〈 0.0001) proved independent. The degree of treatment response was dependent on the size of CNV surface (p=0.05) and flow area (p=0.05) on OCTA in the univariate analysis, and the latter independent after multivariate analysis. In addition, a shorter time period of retinal fluid appeared to play a role (p=0.01 multivariate, p=0.19 univariate). Conclusion The anti-VEGF response was highly variable and often incomplete, suggesting that CNV was not solely responsible for the fluid accumulation. Predictive factors may guide indication for anti-VEGF in CNV associated with CSC.

    Abstract: To describe recurrence patterns and investigate candidate risk factors for recurrences of central serous chorioretinopathy. Methods: In 46 patients with acute central serous chorioretinopathy and follow-up 〉 12 months after first episode resolution, parameters influencing recurrences were retrospectively evaluated using a frailty Cox proportional hazard survival model. Covariates included baseline systemic findings: age, gender, corticosteroid use, stress, shift work, sleep disorder, depression, allergy, cardiovascular risk; baseline optical coherence tomography findings: subfoveal choroidal thickness, pigment epithelial detachment pattern (regular/bump/irregular), number of subretinal hyperreflective foci at leakage site; baseline angiographic findings: fluorescein leakage intensity (intense/moderate/subtle/absent), hyperpermeability pattern on indocyanine-green angiography (focal/multifocal); and episode-related findings: duration and treatment of previous episode. Results: Twenty of 46 subjects (43%) presented ≥1 recurrences during a mean follow-up of 29.9 ± 9.5 months (range, 15–54 months). Follow-up duration did not differ between cases with or without recurrences ( P = 0.3). Worse final visual acuity levels (logarithm of the minimal angle of resolution) were associated with a higher number of episodes during follow-up ( P = 0.032, r = 0.28). In a univariate analysis, higher subfoveal choroidal thickness ( P = 0.021), nonintense fluorescein leakage (= moderate/subtle/absent, P = 0.033), multiple subretinal hyperreflective foci ( P = 0.026), and shift work ( P 〈 0.0001) were significantly associated with recurrences, with a near-significant influence of irregular pigment epithelial detachment ( P = 0.093). In a multivariate analysis, higher subfoveal choroidal thickness ( P = 0.007), nonintense fluorescein leakage ( P = 0.003) and shift work ( P 〈 0.0001) remained significant and independent risk factors for recurrences. Conclusion: Multiple factors influence the risk of central serous chorioretinopathy recurrence. These findings may contribute to identify patients at higher risk, who could benefit from earlier or more intensive treatment.

    Abstract: To evaluate the laterality of Coats disease by analyzing optical coherence tomography angiography features in affected, fellow, and control eyes. Methods: Patients with Coats disease who underwent optical coherence tomography angiography were retrospectively reviewed. Healthy eyes of age-matched patients served as controls. Automated optical coherence tomography angiography determination of foveal avascular zone size and vascular density of superficial capillary plexus and deep capillary plexus was recorded. Results: Thirty-four patients with Coats disease (13 with bilateral optical coherence tomography angiography) and 24 controls were included. The foveal avascular zone was larger in affected eyes compared with fellow eyes ( P = 0.004). Vascular density was decreased in affected eyes compared with fellow eyes in the superficial capillary plexus and deep capillary plexus whole images ( P = 0.047 and P = 0.007) and in the deep capillary plexus at the fovea ( P = 0.001). Vascular density was significantly reduced only in the deep capillary plexus in Stage 1 or 2A patients but in both plexuses in patients with Stage 2B1. No differences were shown on foveal avascular zone and vascular density values between fellow eyes of patients with Coats disease and controls. Conclusion: The foveal avascular zone is enlarged, and vascular density is decreased in affected eyes with Coats disease, but no differences are seen between fellow and control eyes, confirming the unilateral nature of the disease.

    Abstract: Анотація. У статті лірику Івана Гнатюка розглянуто в координатах національного характеру екзистенції. Її мета полягає в окресленні й характеристиці домінантних модусів художньої парадигми поезії митця як концептуально-філософських основ його поетичної творчості. У розвідці застосовано комплексний підхід з використанням методів аналізу, синтезу, також зіставного й герменевтичного. Комплексна методологія дала можливість визначити та виокремити низку домінантних модусів художньої парадигми Івана Гнатюка, дослідити специфіку їх літературної інтерпретації автором, розглянути прояв деяких положень філософії екзистенціалізму крізь духовні координати особистості. Це допомогло проаналізувати поетичний доробок Івана Гнатюка в площині національного характеру екзистенції – у цьому наукова новизна розвідки. Поезія Івана Гнатюка змушує замислитися над буттям окремішньої особистості, призначенням та змістом життя. Світогляд поета, його доля відбивають національну самобутність, укоріненість та історичну свідомість рідного народу. Ліричний герой Івана Гнатюка плекає своє духовне осердя – Україну, у його поетичному слові вирує національний дух. Мистецько-художнім осмисленням явищ нашої історії та сучасності Іван Гнатюк утверджує непроминущі загальнолюдські, національні, історичні, моральні, естетичні, духовні цінності. Людина й народ постають у його віршах єдиним цілим, їхній взаємозв’язок та взаємозалежність допомогли авторові створити особливу художню концепцію людини як особистості національної, розкрити її істинну сутність. Поет виводить узагальнений образ нації як унікального духовного світу, створеного «згустками буття» неповторних індивідуумів. Встановлено, що визначальне місце в поетичному доробку Івана Гнатюка посідають три абсолютні константи: Бог, Україна, Свобода як модуси національно-екзистенційної інтерпретації творчості поета. Свобода в поезії Івана Гнатюка постає тією категорією, що формує провідну структуру національної екзистенції. Її поет зображає не як кінцеву мету, а як споконвічний шлях. Іван Гнатюк упродовж усього життя був вірний провідним мотивам своєї творчості: відданість українському родові, рідній мові, пам’яті про рідну землю, саме вони стимулювали усі його творчі й життєві порухи.
