Nierenzellkarzinome (NZK) werden aufgrund fortgeschrittener bildgebender Verfahren früher erkannt und können heutzutage oftmals kurativ operiert werden, überwiegend sogar unter Erhaltung der tumorfreien Restniere. Ferner stehen den behandelnden Ärzten erstmals zielgerichtete Therapeutika in der Behandlung des fortgeschrittenen NZK zur Verfügung. Trotz dieser positiven Entwicklung mit sinkender Nierenkrebssterblichkeit fehlen Biomarker, die es ermöglichen, den Verlauf der Erkrankung besser zu prognostizieren, um dann unter Berücksichtigung der Komorbiditäten, Abwägung des Behandlungsrisikos und der molekularen Tumorcharakteristika eine für den Patienten maßgeschneiderte Therapie einzuleiten. Wir konnten zeigen, dass Proteine, die regulatorisch auf apoptotische Signalkaskaden wirken, sowohl vielversprechende prognostische Biomarker, aber auch interessante Zielstrukturen für neue Therapieansätze darstellen. Ferner zeigen unsere Untersuchungen, dass molekulare Tests erforderlich sind, um eine korrekte Tumorklassifizierung eines NZK mit Xp11.2-Translokation zu gewährleisten, da neben Translokationen auch Amplifikationen zur Aktivierung von TFE3-Gens in NZK führen können. Die translationale Forschung mit Identifikation und Etablierung von Biomarkern sowie die Subtypisierung und molekulare Charakterisierung von NZK ist Voraussetzung für eine erfolgreiche zielgerichtete Therapie des NZK.
