Einleitung: Die Versorgungsstrategie der SLAP(superior labrum anterior posterior)-Läsion wird generell, jedoch insbesondere bei Patienten über 40 Jahren, kontrovers diskutiert. Unterschiedliche Einflussfaktoren wie u. a. die zunehmende Anzahl von Begleitläsionen im höheren Alter spielen im Entscheidungsalgorithmus eine Rolle. Evidenz hinsichtlich der Therapieplanung SLAP-Repair vs. Tenodese/Tenotomie existiert nicht. Ziel dieser Studie war es, im Rahmen eines Kohortenvergleichs die Resultate nach SLAP-Repair bei Patienten unter und über 40 Jahren zu analysieren. Patienten und Methoden: 45 Patienten wurden nach operativer Versorgung einer SLAP-Läsion mit einem mittleren Follow-up von 60 Monaten (5 Jahre) klinisch mithilfe des Constant Score (CS), des Rowe Score und des “Subjective Shoulder Value“ (SSV) nachuntersucht. Davon wurden 18 Patienten der Gruppe 1 (Alter bei OP 〈 40Jahre, mittleres Alter bei OP 29 Jahre, range: 21 − 39 Jahre) und 27 Patienten der Gruppe 2 (Alter bei OP ≥ 40 Jahre, mittleres Alter bei OP 50 Jahre, range: 40 − 60 Jahre) zugeteilt. Ergebnisse: Es zeigte sich ein mittlerer CS von 89 % (Min 16 % − Max 105 %). Der Rowe Score lag durchschnittlich bei 90 Punkten (Min.35 Punkte − Max.100 Punkte), der SSV bei 90 % (Min 20 % − Max 100 %). Im Vergleich beider Gruppen zeigte sich weder im CS (p = 0,198) (Gruppe1: Min 58 % − Max 105 %, Median 92 %; Gruppe 2: Min 16 % Max 105 %, Median 89 %) noch im Rowe Score (p = 0,5) (Gruppe 1: Min 55 Punkte − Max 100 Punkte, Median 85 Punkte; Gruppe 2: Min 35 Punkte − Max 100 Punkte, Median 92,5 Punkte) ein signifikanter Unterschied. Im SSV zeigte sich ebenfalls kein Unterschied auf signifikantem Niveau (p = 0,068) zwischen beiden Gruppen (Gruppe 1: Min 60 % − Max 100 %, Median 95 %; Gruppe 2: Min 20 % − Max 100 %, Median 90 %), wobei der SSV in Gruppe 1 eine bessere Korrelation mit dem CS aufwies als in Gruppe 2. Diskussion: Das Patientenalter scheint weniger Einfluss auf das Ergebnis nach SLAP-Rekonstruktion zu haben als bislang angenommen. Auch im Alter ≥ 40 Jahre zeigt die Rekonstruktion des SLAP-Komplexes gute klinische Resultate, die mit den bekannten Literaturdaten vergleichbar sind. Einfluss auf das Langzeitergebnis scheint insbesondere die Anzahl und Schwere der Begleitverletzungen, nicht das Patientenalter zu haben.
