En particular, en las dos últimas décadas, la toma de decisiones en el ámbito de la salud ha tenido que adaptarse a un mundo cambiante, tanto a nivel de las tendencias políticas (con una disminución del papel del Estado en favor de un mercado en funcionamiento libre y competitivo) como desde el punto de vista de las restricciones financieras. La decisión sobre la afectación y distribución de recursos ha sido, por tanto, escenario de un intenso debate en la arena académica y política. Sin embargo, las decisiones sobre la naturaleza, el alcance y la distribución de los recursos que deben aportarse son complejas, implicando, no solo criterios técnicos, sino también juicios de valor y la creación de consensos políticos. Esta cuestión es aún más apremiante en una coyuntura de contención de gastos, en la que la demanda de una utilización más racional de recursos (eficiencia) gana mayor relieve, provocando cierta tensión con el principio de equidad. La influencia de la dimensión territorial en los procesos de toma de decisiones en salud ha adquirido, a la luz de este contexto, una atención creciente en el pasado reciente.
