En 2008, les progrès des connaissances en matière de carcinogenèse ont permis d'identifier des cibles moléculaires clés dans la genèse de nombreuses tumeurs solides. Bien qu'encore limitées, ces informations permettent l'emploi de composés dirigés contre ces cibles moléculaires, créant ainsi le concept de thérapie ciblée. Le trastuzumab, dirigé contre HER2, et l'imatinib, inhibiteur de kinase dirigé contre la protéine de fusion bcr/abl, ont été les modèles. Plusieurs anticorps ou molécules inhibitrices de tyrosine kinase sont déjà commercialisés. Ces traitements donnent parfois des taux de réponse limités en monothérapie, mais ont souvent une activité antitumorale additive avec la chimiothérapie dans plusieurs types de tumeurs. Ces nouvelles classes thérapeutiques vont continuer à se développer. Grâce à elles, des maladies telles que le cancer du rein, du côlon ou du sein, les GIST, ont une histoire naturelle désormais modifiée.
