Seit tiber 100 Jahren werden Nitrovasodilatatoren in der Therapie der myokardialen Ischamie verwendet. Die Bedeutung der Endothelzellen bei der Entwicklung koro narer GefaBkrankheiten konnte erst in den letzten 2 Jahrzehnten in ihrer pathophy siologischen Bedeutung und den daraus resultierenden pharmakologischen Rationa len genauer aufgekliirt werden. Endothelzellen sind in der Lage, eine Reihe von Faktoren freizusetzen, die die Funktion von glatten GefaBmuskelzellen und von Thrombozyten regulieren. Von den vielen Faktoren ist das Stickstoffmonoxid (NO) besonders wichtig. Diese Substanz besitzt antiaggregatorische, antiproliferative und vasodilatorische Eigenschaften. Gegenspieler des Endothelium derived nitric oxyd (EDNO) ist das Endothelin-I (ET-I), welches vasokonstriktorische und proliferative Effekte besitzt.

Der Tonus der fUr die Entwicklung der koronaren Herzkrankheit verantwortlichen groBen epikardialen Arterien wird von endothelialen, im Zusam menspiel mit humoral-neurohormonalen Faktoren reguliert. Bei der koronaren Herz krankheit ist dieser Regulationsmechanismus dahingehend gest6rt, daB die von der Endothelstruktur und -funktion abhangige NO-Ausschiittung vermindert ist. Daher tiberwiegt die myogene konstriktorische Aktivitiit. Bei geschiidigter Endothelfunkti on kommt es hierdurch zu verminderter myokardialer Perfusion und Sauerstoffver sorgung. Eine Therapie mit exogen zugefiihrten Nitrovasodilatatoren kommt daher die Bedeutung einer Substitutionstherapie zu. Allerdings fiihrt eine langfristige, nicht-intermittierende Verabfolgung vasodilatorischer Substanzen im Rahmen der Angina pectoris zu einer Wirkungsabschwachung, Toleranzentwicklung. Diese To leranz stellt ein multifaktorielles Phanomen dar.
