Dans la seconde moitié du XXe siècle, l'activité biomédicale s'est largement déployée face aux processus d'usure biologique accompagnant le vieillissement et conduisant au décès. La fréquence et la durée croissante des hospitalisations, les passages en réanimation, la dépendance aux techniques instaurées et leur poursuite à domicile ou en institution, transforment profondément l'environnement du sujet et son insertion sociale. Cette transformation et ses répercussions psychiques sont négligées par l'approche biomédicale, et c'est l'enjeu initial de cette thèse que de faire progresser l'interprétation des trajectoires et des tensions ainsi produites. Pour ce faire, divers concepts, dont celui de statut social, doivent être travaillés et redéfinis afin de mieux prendre en compte les objets, les appareils, les réseaux et les dispositifs techniques dans l'environnement (social) du sujet. Le cas des insuffisants respiratoires chroniques permet une exploration en quelque sorte exemplaire qui nourrit cette adaptation conceptuelle. A un niveau fondamental, celle-ci conduit à ébaucher une théorie de la médiation du corps dans la construction des univers symboliques (institués/intériorisés). A un niveau plus conjecturel, en rapport direct avec les pratiques explorées, elle contribue à préciser une modélisation de l'ordre social, concret et symbolique, intégrant l'essor des dispositifs techniques

In the second half of the twentieth century, biomédical activity has been widely developed facing biological wear processes linked to ageing and leading to death. Hospitalization increasing in frequency and length, stays in intensive care, addiction to the techniques used and their continuation at home or in institutions, deeply change people's environment and their social insertion. This transformation and its psychological consequences are neglected by the biomédical approach and the initial challenge of this thesis is to progress in the interpretation of trajectories and tensions thus produced. For that purpose, several concepts, one being social status, must be reconsidered and redefined in order to take into account better the objects, apparatus, networks and technical systems in people's (social) environment. The case of people with chronic respiratory insufficiency allows an exploration in a sense exemplary which sustains this conceptual adaptation. At a basic level, this adaptation leads to sketch a theory of the mediation of the body in the construction of symbolic universes (established/interorised). At a more circumstantial level, directly connected with the explored practices, it contributes to define a modelization of social order, concrete and symbolic, integrating the development of the technical systems
