Content: Acute kidney injury is defined as an abrupt change in serum creatinine and/or urine output, and a majority of patients admitted to the ICU have some evidence of the disorder. Unfortunately, treatment for this complex syndrome is as yet lacking and understanding is limited. An interdisciplinary panel of experts has contributed to this volume, illuminating some of the fundamental and complex aspects of the disorder ranging from pathophysiology to treatment, from emerging biomarkers to genetic polymorphisms. Other contributions focus on immunological issues or the many complications of acute kidney injury and co-morbid conditions encountered, covering the fundamentals as well as the latest developments. Moreover, important technical aspects of extracorporeal therapies including vascular access, anticoagulation or fluid composition are introduced, and different approaches to renal support from intermittent dialysis to continuous therapies and hybrid techniques are discussed. A description of advanced extracorporeal techniques of organ support and their role in the management of sepsis and acute kidney injury in the context of an overall strategy of multi-organ failure management concludes the discussions. This volume not only provides a practical and up-to-date summary of current knowledge and technology, but also imparts a fundamental understanding of the pathogenesis and likely future developments in this field. It also serves to challenge and re-examine the fundamental underlying assumptions we hold regarding critical illness in general and acute kidney injury in particular
Note: an assessment of current knowledge and future developments

Content: While continuous ambulatory peritoneal dialysis (CAPD) has been the standard peritoneal procedure since the seventies, different schedules of automated peritoneal dialysis (APD) have emerged during the eighties. Today, APD is considered a valuable tool in the management of ESRD patients, together with CAPD and hemodialysis. However, despite its frequent use, APD has § not yet been well assessed, and most pathophysiological and clinical studies on PD refer to CAPD. In this book, major experts in the field therefore discuss and evaluate the insights gained on APD up to now, presenting a comprehensive review of all experimental, technical and clinical aspects related to the various treatments grouped under the definition of APD. The recent developments presented are divided into four sections: membrane permeability, transport mechanisms and kinetic modeling applied to APD; prescription and adequacy of different APD treatment schedules; dialysis machines and solutions for APD, and, lastly, different clinical aspects such as the possibility to maintain APD program and residual renal function. Physicians involved in ESRD care, renal fellows and scientists both in the academic world and in the hospital setting will undoubtedly profit from this timely publication

Content: Sodium hypochlorite has long been recognized for its effectiveness as an antiseptic and disinfectant, but its practical use in medicine had been limited due to its reduced stability. However, it has been shown that the method of manufacture can make the vital difference: The ExSept solutions (Amuchina) discussed in this publication differ from other commercially manufactured solutions insofar as they are produced with the help of electrolysis, the result being a highly effective antimicrobial with very good biocompatibility. The papers in this volume concentrate on the experiences made with ExSept solutions in dialysis applications. They provide a sound understanding of how electrolytically produced sodium hypochlorite solutions differ from other commercial solutions, present different uses as both an antiseptic and disinfectant and alleviate some of the stereotypes associated with their medical use. Both in vitro controlled laboratory evaluations and clinical in vivo prospective randomized trials are introduced in this publication, making it a valuable help for clinicians working in the field of dialysis

Content: This book surveys experiences made with Polymethylmethacrylate (PMMA) dialysis membranes, ranging from a summary of the most recent experimental and clinical results to an overview of future potentials and new trends in research. Several articles treat the biophysical properties of the membrane, its chemical structure and manufacturing procedures aimed at obtaining hollow fibers suitable for medical purposes. Moreover, various experimental conditions are considered in order to identify the dialytic properties of the PMMA membrane and dialyzer series. In this context, particular emphasis is put on the last generation of PMMA membranes with higher porosity and ultrafiltration capacity. On the more practical side, an overview is given of various clinical conditions in which PMMA membranes are utilized at variable rates of ultrafiltration, blood and dialysate flows. The bio- or hemocompatibility of PMMA membranes is investigated, offering a summary of the blood-membrane interactions during hemodialysis. Furthermore, the importance of these aspects in relation to the chronic dialyzed and the acutely ill patients is highlighted, as well as long-term experiences made with different PMMA membranes. For those interested in the last developments and clinical results achieved with synthetic membranes, this book provides a substantial piece of information valuable either for further research or daily clinical practice

Content: In recent years, major concerns have been expressed about long-term derangements in hemodialysis patients and possible clinically relevant blood-membrane interactions. Preliminary studies employing biocompatible membranes and antioxidant agents such as vitamin E have demonstrated significant improvements in patient organ function and hemopoietic capacity with increased hematocrit and quality of life. Using a new technology, vitamin E has now been successfully bound to the inner surface of a hollow-fiber membrane, so that the patient’s blood is flowing in contact with a layer of antioxidant agent covering the inner surface of the membrane in the hemodialyzer. The opening chapters of this publication give an overview on oxidant stress and the role of vitamin E as antioxidant agent and elaborate on technical aspects, hydraulics and permeability characteristics of the membrane in question. The main part is dedicated to the hemocompatibility of the membrane, its effect on erythrocyte morphology and survival and the impact on white cell and platelet function. The metabolic effects and the antioxidant impact of the vitamin E-coated membrane are presented in the last part of the book.Intended as a reference tool for professionals, students and fellows involved in the care of end-stage renal disease patients, it is also hoped that this book serves as a starting point for further speculations and research in the field

Content: The presence of endotoxin in the bloodstream is the consequence of an immunological response triggered by bacterial invasion of the host. This condition leads to altered cardiovascular function, lung dysfunction and acute kidney injury. Removing as much as possible of the circulating endotoxin is therefore imperative to mitigate its biological and clinical effects at the cellular, tissue and organ levels. The publication at hand summarizes the basic mechanisms, rationale and clinical results of a new therapeutic approach that promises to attain this goal: It consists of a specific hemoperfusion process that utilizes cartridges with immobilized polymixin B in an extracorporeal circuit.Containing contributions by renowned researchers, this book represents as a milestone in the field of extracorporeal therapies in sepsis. It will be a valuable reference work for both basic scientists and clinical professionals who want to keep abreast of current developments

Content: Over the last decade, the availability of new devices and membranes has led to the development of modern hemodialysis techniques in which clinical tolerance and efficiency have significantly improved patient morbidity and mortality. Nevertheless, there are still limitations imposed by hemodialysis, manifested by the long-term complications occurring in patients treated for several years.Since most of these complications seem to be related to the chronic inflammation of ESRD patients and the progressive accumulation of middle-large molecules such as beta-2-microglobulin, GIP, leptin and others, a novel approach is needed, capable of adequately removing these substances from the body of the patient. New polymers with a biocompatible surface and high affinity for specific molecules have already been introduced to the market, and adsorption might in fact represent a new form of solute removal.Featuring articles by renowned experts, this volume provides the reader with a comprehensive summary of the most updated technology in the field of hemodialysis. New devices and techniques are described and discussed in detail, complemented by preliminary results; the rationale for adsorptive therapies completes the analysis. Residents and fellows, as well as staff physicians and members of the academic and scientific community involved in practice and research in the field of hemodialysis will find this book a valuable addition to their bookshelves
