    Description: Effects of reactive red 2 and its parent compound acid red 33 were studied in rat vas deferens and guinea-pig taenia coli. In rat vas deferens, reactive red 2(1 to 10 microM) shifted the concentration-response curve for the P2X-purinoceptor-mediated contraction effect of alpha, beta-methylene ATP slightly to the right and progressively decreased the maximum (apparent antagonist Kd value 0.42 microM). Acid red 33 (1000 microM) shifted the curve to the right without changing the maximum (apparent Kd 386 microM). The concentration-contraction curve of noradrenaline was not altered by reactive red 2. In the carbachol-precontracted guinea-pig taenia coli, reactive red 2 (0.1 to 10 microM) shifted the concentration-response curve for the P2Y-purinoceptor-mediated relaxation effect of adenosine 5'-O-(2-thiodiphosphate) (ADP beta S) progressively to the right; only at the highest concentration of antagonist (10 microM) was the maximum slightly depressed; a pA2 value of 7.55 (Kd 0.028 microM) was derived from the shift. Acid red 33 (1000 microM) shifted the concentration-relaxation curve of ADP beta S to the right without changing the maximum (apparent Kd 171 microM). Reactive red 2 (1 to 10 microM) also shifted the concentration-response curve for the relaxation effect of alpha, beta-methylene ATP, which is mediated by an unclassified P2-purinoceptor, progressively to the right but simultaneously decreased the maximum (apparent Kd 1.6 microM). The concentration-relaxation curve of 2-chloroadenosine was not altered by reactive red 2. Pieces of vas deferens and taenia coli degraded 76 and 66% of added ATP (10 microM) within 30 min, respectively. Reactive red 2(0.1 to 100 microM) progressively reduced this degradation by up to 95%, with IC50 values of 3.9 +/- 0.6 and 3.9 +/- 2.3 microM, respectively. Acid red 33 (1000 microM) reduced the degradation by 30 and 20%, respectively. The results indicate that reactive red 2 is a relatively potent antagonist at both P2X-purinoceptors in rat vas deferens and P2Y-purinoceptors in guinea-pig taenia coli, with a 15 fold selectively for the P2Y-purinoceptor. It inhibits ecto-nucleotidase in both tissues. The dichloro-triazine residue that distinguishes the compound from acid red 33 greatly enhances the potency at both receptor subtypes as well as at the nucleotidase. As regards P2-purinoceptor subtypes, the results confirm the existence of two relaxation-mediating P2-purinoceptors in guinea-pig taenia coli.

Abstract Objective Functional somatic syndromes (FSS), defined as physical syndromes without known underlying organic pathology, are sometimes regarded as less serious conditions than well-defined medical diseases (MD). The aims of this study were to evaluate functional limitations in FSS, and to compare the results to MD patients with the same core symptoms. Methods This study was performed in 89,585 participants (age: 44.4[plus or minus]12.4 years, 58.5% female) of the general-population cohort LifeLines. Quality of Life (QoL) and work participation were examined as indicators of functional limitations. QoL was assessed with two summary scales of the RAND-36: the physical component summary (PCS) and the mental component summary (MCS). Work participation was assessed with a self-reported questionnaire. QoL and work participation were compared between FSS and MD patients, using Chi-squared tests and ANCOVA-analyses, adjusted for age, sex, educational level, and mental disorders. Results Of the participants, 11.0% (n=9861) reported a FSS, and 2.7% (n=2395) reported a MD. Total QoL, PCS and MCS were significantly lower in all separate FSS and MD compared to controls (P[less than or equal to] .001). Clinically relevant differences in QoL were found between chronic fatigue syndrome and multiple sclerosis patients, and between fibromyalgia syndrome and rheumatoid arthritis patients. Compared to controls, FSS and MD patients reported a comparably reduced working percentage, increased sick absence, early retirement due to health-related reasons, and disability percentage (P[less than or equal to] .001). Conclusion Functional limitations in FSS patients are common, and as severe as those in patients with MD when looking at QoL and work participation, emphasizing that FSS are serious health conditions. Methods From our pool of clinical pediatric MRI examinations at 3 T all patients with normal conventional MRI scans were retrospectively selected. Depending on their clinical findings the identified 99 patients (0--199 months) were divided into 43 healthy controls and 56 diseased children with various clinical abnormalities (developmental delay, epilepsy, prematurity, and deafness). T.sub.2 maps based on routinely performed triple echo turbo spin echo sequences were created. T.sub.2 values were measured in 22 brain regions to determine age-related changes. We also investigated whether such changes differ between healthy and diseased children. Results Age significantly reduced T.sub.2 relaxation times across all regions (p 〈 0.05), but health status had no impact. With increasing age, T.sub.2 values decreased continuously, with declines faster over the first 10 months and slower thereafter. Early rapid and later slow decline was similar in healthy and diseased groups. Conclusions Using T.sub.2 maps based on clinical MRI data we could determine age-related T.sub.2 relaxation times in 22 brain regions during infancy and childhood. Our data have relevance for future investigator independent T.sub.2 relaxation time measurements in determining whether T.sub.2 values are within the normal range or should be considered as potentially pathologic.     Description: The objectives of this study were to (1) examine whether need for recovery differs between workers (i) not on-call, (ii) on-call but not called and (iii) on-call and called, and (2) investigate the associations between age, health, work and social characteristics with need for recovery for the three scenarios (i–iii). Cross-sectional data of N = 169 Dutch distal on-call workers were analysed with multivariate logistic regression. Need for recovery differed significantly between the three scenarios (i–iii), with lowest need for recovery for scenario (i) ‘not on-call’ and highest need for recovery for scenario (iii) ‘on-call and called’. Poor mental health and high work–family interference were associated with higher need for recovery in all three scenarios (i–iii), whereas high work demands was only associated with being on-call (ii and iii). The results suggest that the mere possibility of being called affects the need for recovery, especially in workers reporting poor mental health, high-work demands and work–family interference.

    Description: Current work and health research is fragmented, focusing on jobs, exposures, specific worker groups, work organization, or employment contracts. An emphasis on the labor market in framing the work and health relationship conceptualizes work not only as an exposure that increases or lessens health risk but also as a life course experience that is dependent on place and time. The intention is to illustrate how the labor markets and health framework coupled with a life course perspective extends other epidemiological approaches to work and health to identify new research questions. Taking the changing nature of work and labor markets into account, this paper updates the labor markets and health framework. It then reviews, defines, and integrates key life course concepts. A model is developed that guides the understanding of how labor markets and health trajectories emerge from the consideration of the working life course in a social context. The application leads to new research questions investigating characteristics of labor markets and health trajectories that may lead to positive health outcomes over the life course.

    Description: OBJECTIVES: To examine the employment situation and predictors of return to work for individuals with spinal cord injury 5 years after discharge from inpatient rehabilitation.DESIGN: Prospective cohort study.SUBJECTS: A total of 114 subjects who were employed before the injury and who completed a 5-year follow-up.METHODS: Work was defined as having paid work ≥ 1 h/week or ≥ 12 h/week. Predictors of return to work were identified using logistic regression analysis. Demographic, injury-related, pre-injury work factors and self-efficacy were measured at the start of rehabilitation and at discharge.RESULTS: Return to work rates for ≥ 1 and ≥ 12 h/week were 50.9% and 42.6%, respectively. Median time to return to work was 13 months. Compared with before injury, participants worked for fewer hours per week and had occupations of lower physical intensity. The majority had a supplementary income. Those who returned to work were financially better-off than those who did not. Only 40% of participants received return to work support. A high/middle level occupation was associated with higher odds of return to work ≥ 1 h/week (odds ratio (OR) = 2.39, 95% confidence interval (95% CI) = 1.07-5.30). Low physical intensity of pre-injury occupation was significantly associated with higher odds of return to work ≥ 1 h/week (OR = 3.01, 95% CI = 1.31-6.91) and ≥ 12 h/week (OR = 2.67, 95% CI = 1.18-5.96). After adjustment for potential confounders, these associations were no longer significant.CONCLUSION: Return to work after spinal cord injury was relatively high in this study, but entailed considerable changes in the employment situation, especially reduced working hours and less physically intense occupations. Rehabilitation interventions should enhance the skills and qualifications of individuals with physically-demanding pre-injury work in order to improve access to suitable jobs after spinal cord injury. Interventions should focus not only on return to work, but also on the quality of employment, including opportunities to pursue full-time work.

    Description: Glycodelin [gene name, progesterone associated endometrial protein (PAEP)] was initially described as an immune system modulator in reproduction. Today, it is also known to be expressed in several types of cancer, including non-small cell lung cancer (NSCLC). In this cancer type, the feasibility of its usage as a follow-up biomarker and its potential role as an immune system modulator were described. It is assumed that NSCLC tumours secrete glycodelin to overcome immune surveillance. Therefore, targeting glycodelin might be a future approach with which to weaken the immune system defence of NSCLC tumours. In this context, it is important to understand the regulatory pathways of PAEP/glycodelin expression, as these are mostly unknown so far. In this study, we analysed the influence of several inducers and of their downstream pathways on PAEP/glycodelin expression in a human lung adenocarcinoma carcinoma (ADC; H1975) and a human lung squamous cell carcinoma (SQCC) cell line (2106T). PAEP/glycodelin expression was notably stimulated by the canonical transforming growth factor (TGF)-[beta] pathway in SQCC cells and the PKC signalling cascade in both cell lines. The PI3K/AKT pathway inhibited PAEP/glycodelin expression in the ADC cells and an antagonizing role towards the other investigated signalling cascades is suggested herein. Furthermore, the mitogen-activated protein kinase kinase (MEK)/extracellular-signal regulated kinases (ERK) pathway was, to a lesser extent, found to be associated with increased PAEP/glycodelin amounts. The phosphoinositide 3-kinase (PI3K)/protein kinase B (AKT), MEK/ERK pathway and TGF-[beta] are targets of NSCLC drugs that are already approved or are currently under investigation. On the whole, the findings of this study provide evidence that inhibiting these targets affects the expression of glycodelin and its immunosuppressive effect in NSCLC tumours. Moreover, understanding the regulation of glycodelin expression may lead to the development of novel therapeutic approaches with which to weaken the immune system defence of NSCLC tumours in the future.

    Description: The aim of this study was to prospectively investigate the effect of potential workplace closure, an externally attributed form of job insecurity, on fatigue and psychological distress. In April 1999, the minister responsible announced that a governmental agency would close down one of its two locations. Baseline data from the employees were available before the closure announcement (n = 574) because they were enrolled in the Maastricht Cohort Study on "Fatigue at Work." In the closure group, the relative risk for becoming a psychological distress case was 1.61 (95% confidence interval [CI] = 1.27-2.05) during 13 months after the closure threat. Within the closure group, a difference in relative risk for psychological distress was observed between employees who self reported an increase in job insecurity (relative risk = 1.85; 95% CI = 1.41-2.42) and employees who did not report an increase in job insecurity (relative risk 1.14; 95% CI = 0.58-2.24).
