Content: Acute kidney injury is defined as an abrupt change in serum creatinine and/or urine output, and a majority of patients admitted to the ICU have some evidence of the disorder. Unfortunately, treatment for this complex syndrome is as yet lacking and understanding is limited. An interdisciplinary panel of experts has contributed to this volume, illuminating some of the fundamental and complex aspects of the disorder ranging from pathophysiology to treatment, from emerging biomarkers to genetic polymorphisms. Other contributions focus on immunological issues or the many complications of acute kidney injury and co-morbid conditions encountered, covering the fundamentals as well as the latest developments. Moreover, important technical aspects of extracorporeal therapies including vascular access, anticoagulation or fluid composition are introduced, and different approaches to renal support from intermittent dialysis to continuous therapies and hybrid techniques are discussed. A description of advanced extracorporeal techniques of organ support and their role in the management of sepsis and acute kidney injury in the context of an overall strategy of multi-organ failure management concludes the discussions. This volume not only provides a practical and up-to-date summary of current knowledge and technology, but also imparts a fundamental understanding of the pathogenesis and likely future developments in this field. It also serves to challenge and re-examine the fundamental underlying assumptions we hold regarding critical illness in general and acute kidney injury in particular
Note: an assessment of current knowledge and future developments

Content: The pathological role of advanced glycation end products (AGEs) and of oxidative-carbonylic stress is well known in the context of diabetes. Moreover, there is also strong evidence that they play a pivotal role in the pathogenesis of some chronic disorders related to uremia, and namely to cardiovascular complications and dialysis-related amyloidosis. They may even play a crucial role in the loss of function of the peritoneal membrane in patients on peritoneal dialysis, a problem which limits the duration of such a treatment.Written by some of the most authoritative European experts, this publication includes up-to-date contributions on AGEs as related to diabetic nephropathy, hemodialysis and, particularly, peritoneal dialysis. The topics covered range from basic research (e.g. novel pathogenic pathways triggered by AGEs) to clinical issues, including substitutive therapy in uremic diabetic patients as well as original and new results from a clinical trial on peritoneal dialysis with a new bag. This publication will be of interest to nephrologists as well as those working in the field of diabetes or otherwise involved in AGE research, giving an idea into what direction research in this complex field is going and which will probably be the major clinical breakthroughs in the near future

Content: While continuous ambulatory peritoneal dialysis (CAPD) has been the standard peritoneal procedure since the seventies, different schedules of automated peritoneal dialysis (APD) have emerged during the eighties. Today, APD is considered a valuable tool in the management of ESRD patients, together with CAPD and hemodialysis. However, despite its frequent use, APD has § not yet been well assessed, and most pathophysiological and clinical studies on PD refer to CAPD. In this book, major experts in the field therefore discuss and evaluate the insights gained on APD up to now, presenting a comprehensive review of all experimental, technical and clinical aspects related to the various treatments grouped under the definition of APD. The recent developments presented are divided into four sections: membrane permeability, transport mechanisms and kinetic modeling applied to APD; prescription and adequacy of different APD treatment schedules; dialysis machines and solutions for APD, and, lastly, different clinical aspects such as the possibility to maintain APD program and residual renal function. Physicians involved in ESRD care, renal fellows and scientists both in the academic world and in the hospital setting will undoubtedly profit from this timely publication

Content: In the last twenty years, critical care medicine has been established as a specialty with its own therapies and procedures, with significant implications for clinical nephrology regarding severe acute renal failure. This typically multi-factorial condition is today predominantly seen in intensive care units. The complex knowledge and skills necessary to handle it have resulted in a field called Critical Care Nephrology, where nephrologist and intensivist either work side by side or have formally acquired expertise and training in both specialties. Extracorporeal renal replacement therapies, although originally used to treat end-stage renal disease patients, rapidly gained importance for acute patients. This has had far-reaching implications for the initiation and possible role of blood purification in the management of critically ill patients, widening the scope of interaction between the nephrologist and the intensivist.Experts in the field have contributed to the present book, providing a comprehensive review of the different blood purification techniques relevant in the intensive care unit in the case of multiple organ failure. The resulting mix of established knowledge and recent results from both clinical trials and basic research constitutes a valuable tool for all professionals involved in the care of the critically ill patient

Content: Chronic kidney disease (CKD) is an established risk factor for cardiovascular diseases. Stroke, on the other hand, is not only a major player in cardiovascular disease, but it also has strong two-way relationships with CKD. Moreover, subclinical cerebral abnormalities are also associated with CKD. But despite all these connections, the cerebro-renal interaction has so far not received much attention. This book includes easily understandable reviews on brain, stroke and kidney by both experts in nephrology and neurology. Examined are underlying concepts for cerebro-renal interaction, risk of clinical and subclinical brain damage in CKD patients, primary prevention and acute/chronic management for stroke patients with CKD and end-stage kidney disease. This book promotes not only further understanding and a multidisciplinary collaboration between nephrologists and neurologists, but it is also of interest for neurosurgeons and cardiologists.

Content: Hemodialysis therapy has become a well-established form of renal replacement, and due to continuous improvements in technology, a series of new biomaterials and techniques for the clinical routine have been developed. Nevertheless, cardiovascular disorders in hemodialysis and uremia represent one of the most important aspects of modern nephrological care. This publication is therefore dedicated to cardiovascular risk factors, the role of the endothelium, problems related to mineral disorders and finally to the possible impact of new technologies on the final outcome in hemodialysis. Being a state-of-the-art appraisal of today’s technology and current issues in hemodialysis with special regard to cardiovascular disorders and risk factors, this book is a synthesis between an educational effort aimed at the newer generations of physicians and nurses in the field of hemodialysis and a state-of-the-art description of the most advanced technologies in this field. At the same time, the lessons learned from large international trials, either already completed or to be completed in the years to come, are presented

Content: This volume presents a unique compilation of reviews on cell volume regulation in health and disease, with contributions from leading experts in the field. The topics covered include mechanisms and signaling of cell volume regulation and the effect of cell volume on cell function, with special emphasis on ion channels and transporters, kinases and gene expression. Several chapters elaborate on how cell volume regulatory mechanisms participate in the regulation of epithelial transport, urinary concentration, metabolism, migration, cell proliferation and apoptosis. Last but not least, this publication is an excellent guide to the role of cell volume in the pathophysiology of hypercatabolism, diabetes mellitus, brain edema, hemoglobinopathies, tumor growth and metastasis, to name just a few.Providing deeper insights into an exciting area of research which is also of clinical relevance, this publication is a valuable addition to the library of those interested in cell volume regulation
