Cryosurgical therapy of liver metastases. First results

Zusammenfassung

□ Patienten und Methodik

An der Klinik und Poliklinik für Allgemein- und Abdominalchirurgie der Johannes-Gutenberg-Universität Mainz wurde zwischen 1. 1. 1996 und 1. 4. 1998 bei 29 Patienten eine Kryotherapie von Lebermetastasen allein oder in Kombination mit einer Resektion in kurativer Zielsetzung durchgeführt.

□ Ergebnis

Nach alleiniger Kryotherapie (n=12) traten keine Komplikationen auf. In der Gruppe der Patienten mit kombiniertem Vorgehen (n=11) verstarb ein Patient an einer Sepsis. Nach Schnittrandvereisung (n=6) kam es je einmal zu einer Gallenfistel und zu einer therapiebedürftigen Aszitesbildung. Von 28 entlassenen Patienten sind bei einer medianen Nachbeobachtungszeit von elf Monaten 15 (53,6%) rezidivfrei. Bei sieben (25%) kam es zu einem Rezidiv der Vereisungszone, bei neun zu einem Wiederauftreten in der Leber und bei drei Patienten zu extrahepatischen Tumorabsiedlungen.

□ Schlußfolgerung

Unter der Voraussetzung, daß eine adäquate Plazierung der Kryosonden zur Sicherstellung einer effektiven Vereisungszone möglich ist, ist die alleinige Kryotherapie eine komplikationsarme, parenchymschonende Alternative zur Resektion von Lebermetastasen und erweitert in Kombination mit der Resektion die operativen Behandlungsmöglichkeiten von Lebermetastasen.

Abstract

□ Patients and Method

Between 1 Jan 1996 and 1 Apr 1998 29 patients under-went cryosurgical therapy for liver metastasis at the Department of Surgery at the University of Mainz.

□ Results

No complications occurred following cryosurgery alone (n=12). Within the group of patients with a combined procedure (n=11) 1 patient had temporary liver failure and 1 patient died of sepsis. Following freezing of the cutting zone (n=6) 1 patient showed a bile fistula and 1 ascites. In 15 cases of 28 dismissed patients with remaining destroyed tumor tissue the follow-up showed no tumor recurrence (median follow-up 11 months). On the other hand 9 patients had a tumor recurrence within the liver, 3 patients at extrahepatic regions and 7 patients developed a tumor recurrence within the freezing zone.

□ Conclusion

Cryotherapy may be regarded as a parenchym sparing and safe alternative to liver resection, if adequate probe placement and an effective freezing zone can be established.
