Samenvatting
Geïsoleerde hooggradige letsels van de nieren na een trauma komen weinig voor in de urologische praktijk. Klinische parameters en radiologisch onderzoek geven inzicht in de ernst van het trauma. De behandelopties voor een nierletsel zijn: conservatief beleid, chirurgisch ingrijpen of een transarteriële embolisatie. Bij de keuze tussen deze behandelingen is hemodynamische (in)stabiliteit van de patiënt de belangrijkste parameter.
Deze casus beschrijft een patiënt met een hooggradig geïsoleerd letsel van de rechternier na een trauma, bij wie conservatieve therapie niet toereikend bleek te zijn. In verband met dreigende hemodynamische instabiliteit werd een transarteriële embolisatie van een bloeding van de rechternier verricht. Aan de hand van deze casus worden de klinische parameters, de classificatie van nierletsels en de behandelopties voor een geïsoleerd traumatisch nierletsel beschreven.
Summary
Isolated traumatic kidney injury: a case report
Severe traumatic kidney injury rarely occurs in isolation. Clinical sings and radiographic imaging can predict severity of the injury. The management of kidney injury is conservative treatment, surgical intervention or transarterial embolization. The therapy depends on the hemodynamic (in)stability of the patient. This case report describes a patient treated with conservative management in a high grade traumatic isolated injury of the right kidney, which needed transarterial embolization because of hemodynamically instability.
In this case report the clinical signs, the classification of kidney injuries and the management after an isolated traumatic kidney injury are described.
