Zusammenfassung
Ein 3-jähriger Junge italienischer Abstammung wird wegen zunehmender körperlicher Mattigkeit, Fieber und gelber Verfärbung der Augen vorgestellt. Der Urin sei dunkel verfärbt. Auf Nachfragen wird der Genuss von »dicken Bohnen« 2 Tage zuvor angegeben. Der Junge ist alters entsprechend entwickelt, fällt aber durch ein blass-ikterisches Hautkolorit auf. Bei der klinischen Untersuchung lässt sich eine grenzwertig vergrö ßerte Milz am Rippenbogen palpieren. Die Herzfrequenz beträgt 160/min, der Blutdruck wird mit 90/60 mmHg gemessen.
Die Laboruntersuchungen ergeben eine Hämoglobinkonzentration von 5,3 g/dl, ein mittleres korpuskuläres Volumen (MCV) von 75 fl, einen mittleren korpuskulären Hämoglobingehalt (MCHC) von 27 pg und einen Retikulozytenanteil von 2,5 %. Das Bilirubin ist auf 3,5 mg/dl mit überwiegend unkonjugiertem Anteil erhöht, ebenso die Laktatdehydrogenase mit 550 U/l. Außerdem ist das Haptoglobin unterhalb der Nachweisgrenze und freies Hämoglobin im Serum nachweisbar. Der Coombs-Test ist negativ.
Der Blutausstrich zeigt eine unauffällige Erythrozytenmorphologie, im Kresylblau-gefärbten Ausstrich werden etliche »Innenkörper« gesehen. Zusätzlich werden im Urin Bilirubin, Urobilinogen und freies Hämoglobin nachgewiesen. Die spezielle Untersuchung der Erythrozytenenzymaktivität en ergibt eine deutliche Aktivitätsminderung der Glukose-6-Phosphatdehydrogenase in den Erythrozyten (vermindert auf weniger als 2 % der Norm).
Aufgrund der Anamnese und der Laborergebnisse kann die Diagnose einer hämolytischen Krise bei Glukose-6-Phosp hatdehydrogenasemangel, ausgelöst durch Nahrungsmittel (»dicke Bohnen«), gestellt werden. Therapeutisch war die Transfusion eines Erythrozytenkonzentrats aufgrund des weiteren Abfalls der Hämoglobinkonzentration auf 4,6 g/dl erforderlich.
