Summary
This is a prospective cohort study comparing transrectal ultrasound (TRUS)-guided 12-core prostate biopsy (standard) to multiparametric magnetic resonance imaging (mpMRI)/TRUS fusion-guided biopsy (targeted) in 1003 men with indications for prostate biopsy. Among the patients, 80.5% had had a prior negative biopsy and patients with a normal mpMRI were excluded. Targeted biopsy of any suspicious lesions was followed by standard random biopsies done by a second operator blinded to the mpMRI
Comment
This study represents by far the largest series in the literature of patients undergoing MR/TRUS fusion biopsy. These results validate many smaller studies in this field that suggest that mpMRI-targeted biopsies identify more intermediate and high-risk disease than standard 12-core biopsy. At the same time, targeted biopsies reduce the detection of clinically insignificant disease.
