Paraneoplastischer oder medikamenteninduzierter subakut kutaner Lupus erythematodes?

Zusammenfassung

Im Januar 2013 stellte sich in unserer Klinik erstmalig eine 72-jährige Patientin mit stammbetonten, großflächigen, zum Teil anulär konfigurierten erythematösen Plaques sowie einer Livedozeichnung im Bereich der unteren Extremitäten vor. Bei der Patientin war 1995 ein Mammakarzinom diagnostiziert worden. Zum Zeitpunkt der Aufnahme bestanden bei der Patientin neu aufgetretene ossäre Filiae. Zudem war der monoklonale Antikörper Denusomab zur Behandlung der Knochenmetastasen begonnen worden. Aufgrund des klinischen Bildes, des histopathologischen Befundes und der charakteristischen Serologie stellten wir die Diagnose eines subakut kutanen Lupus erythematodes. Unter Berücksichtigung des Krankheitsverlaufes und des zeitgleichen Auftretens der Hautveränderungen mit der Entstehung von Metastasen und Einleitung einer Therapie mit Denusomab kann in dem hier präsentierten Fall sowohl das Vorliegen eines paraneoplastischen als auch eines medikamenteninduzierten subakut kutanen Lupus erythematodes diskutiert werden.

Abstract

In January 2013 a 72 year old women presented to our department with widespread, annular, erythematous plaques located on the trunk as well as livedo racemosa located on the lower extremities. In 1995 a diagnosis of breast cancer was made. At the time of in-patient care, new bone metastases were diagnosed. Additionally, treatment with the monoclonal antibody denusomab was initiated for bone metastases. Based on the clinical picture, histopathological analyses, and characteristic serological findings, a diagnosis of subacute cutaneous lupus erythematosus was made. Facing the clinical course of disease with concomitant occurrence of skin lesions and breast cancer metastases as well as beginning of denusomab therapy, the presence of a paraneoplastic or drug-induced subacute cutaneous lupus erythematosus might be discussed.
