Zusammenfassung
Eosinophile Infiltrate sind sowohl bei der akuten als auch chronischen Form der eosinophilen Pneumonie zu finden. Die Ursache dieser Erkrankungsformen kann unbekannt sein, wie bei der chronischen und akuten eosinophilen Pneumonie, der eosinophilen Granulomatose mit Polyangiitis (EGPA) und dem hypereosinophilen Syndrom, oder durch bekannte Ursachen wie Parasiten oder Medikamente ausgelöst werden. In den meisten Fällen ist eine Eosinophilie sowohl im Blut als auch in der bronchoalveolären Lavage (BAL) zu finden. Bei passenden sowohl klinischen als auch radiologischen Befunden gilt die chronisch eosinophile Pneumonie durch eine Eosinophilie über 25 % in der BAL als gesichert. Eosinophile Infiltrate sprechen gut auf Kortikosteroide an, können aber rezidivieren. Im Gegensatz zu der chronischen Form sollte bei der akuten eosinophilen Pneumonie der Eosinophilennachweis in der BAL zwingend erfolgen, da eine Eosinophilie im Blut zu Beginn fehlen kann und somit Fehldiagnosen vermieden werden können. Bei einer Bluteosinophilie, die länger als 6 Monate anhält mit Multiorganbefall und bei Ausschluss bekannter Ursache, ist das hypereosinophile Syndrom als Differenzialdiagnose zu bedenken.
Abstract
Eosinophilic infiltrates are found in both acute and chronic forms of eosinophilic pneumonia. The cause of these forms of the disease may be unknown, as in chronic and acute eosinophilic pneumonia, eosinophilic granulomatosis with polyangiitis (EGPA) and hypereosinophilic syndrome or triggered by known causes, such as parasites and drugs. In most cases, eosinophilia is present in both blood and bronchoalveolar lavages (BAL). With appropriate clinical and also radiological findings, chronic eosinophilic pneumonia is considered to be confirmed by detection of eosinophilia over 25% in the BAL. Eosinophilic infiltrates respond well to corticosteroids but can relapse. In contrast to the chronic form, the detection of eosinophilia in the BAL should be mandatory in acute eosinophilic pneumonia, as eosinophilia in blood may be absent at the beginning and thus misdiagnoses can be avoided. In a blood eosinophilia that lasts more than 6 months with multiple organ involvement and after exclusion of known causes, the hypereosinophilic syndrome has to be considered as a differential diagnosis.
