    Content: Over the past decade, many OECD countries have introduced new policies to tackle excessive waiting times for elective surgery with some success. However, in the wake of the recent economic downturn and severe pressures on public budgets, waiting times times may rise again, and it is important to understand which policies work.  In addition, the European Union has introduced new regulations to allow patients to seek care in other member states, if there are long delays in treatment.   This book provides a framework to understand why there are waiting lists for elective surgery in some OECD countries and not in others. It also describes how waiting times are measured in OECD countries, which differ widely, and makes recommendations for best practice. Finally, it reviews different policy approaches to tackling excessive waiting times. Some countries have introduced guarantees to patients that they will not wait too long for treatment. These policies work only if they are accompanied by sanctions on health providers to ensure the guarantee is met or if they allow greater choice of health-care providers including the private sector. Many countries have also introduced policies to expand supply of surgical services, but these policies have generally not succeeded in the long-term in bringing down waiting times. Given the increasing demand for elective surgery, some countries have experimented with policies to improve priorisation of who is entitled to elective surgery. These policies are promising, but difficult to implement.

    Content: Constructing International Security helps policy makers and students recognize effective third-party strategies for balancing deterrence and restraint in security relationships. Brett V. Benson shows that there are systematic differences among types of security commitments. Understanding these commitments is key, because commitments, such as formal military alliances and extended deterrence threats, form the basis of international security order. Benson argues that sometimes the optimal commitment conditions military assistance on specific hostile actions the adversary might take. At other times, he finds, it is best to be ambiguous by leaving an ally and adversary uncertain about whether the third party will intervene. Such uncertainty transfers risk to the ally, thereby reducing the ally's motivation to behave too aggressively. The choice of security commitment depends on how well defenders can observe hostilities leading to war and on their evaluations of dispute settlements, their ally's security and the relative strength of the defender.

    Content: Although it is fashionable among modernists to claim that globalism emerged only since ca. 1800, the opposite can well be documented through careful comparative and transdisciplinary studies, as this volume demonstrates, offering a wide range of innovative perspectives on often neglected literary, philosophical, historical, or medical documents. Texts, images, ideas, knowledge, and objects migrated throughout the world already in the pre-modern world, even if the quantitative level compared to the modern world might have been different. In fact, by means of translations and trade, for instance, global connections were established and maintained over the centuries. Archetypal motifs developed in many literatures indicate how much pre-modern people actually shared.     Content: Were the English and the Scots always at loggerheads in the fourteenth century? The essays here offer a more nuanced picture. Typical accounts of Anglo-Scottish relations over the whole fourteenth century tends to present a sustained period of bitter enmity, described routinely by stock-phrases such as 'endemic warfare', and typified by battles such as Bannockburn [1314], Neville's cross [1346] or Otterburn [1388], border-raiding and the capture of James I of Scotland by English pirates in 1406. However, as this collection shows, the situation was far more complex. Drawing together new perspectives from new and leading researchers, the essays investigate the great complexity of Anglo-Scottish tensions in this most momentous of centuries and in doing so often reveal a far more ambivalent and at times even a peaceful and productive Anglo-Scottish dynamic. The topics treated include military campaigns and ethos; the development of artillery; the leading 'Disinherited' Anglo-Scot, Edward Balliol; Scots in English allegiance and Border Society; religious patronage; Papal relations; the effect of dealings with Scotland on England's government and parliament; identity, ethnicity and otherness; and shared values and acculturation.
