    Content: 在法医物证实践中,当法医物证检材如血液标本等不具备形态学信息时,根据分子生物学的方法可得到与年龄相关的一些有效信息。已有研究提出,多种生物学指标如端粒DNA限制性酶切片段(TRF)长度、线粒体DNA4977bp片段缺失程度、DNA甲基化水平等,均与个体年龄与衰老密切相关,可能是法医学年龄推断中的有效指标。目前,以上与个体年龄及衰老相关的生物学指标在法医学年龄推断中的应用研究分别取得了不同程度的进展,但大多数仍处于实验阶段,主要的问题是分子生物学的观测指标易受环境、遗传和疾病等的影响,存在着人群和组织器官的差异等;单一指标与年龄之间缺乏精确的定量关系。鉴于此,法医年龄推断研究组(Study Group on Forensic Age Diagnostics)提出,对于每一份检材,尽可能同时使用三种不同的非形态学的生物指标,以提高法医物证检材年龄推断的准确性。因此,寻找更多与年龄相关性高、影响因素少的新指标,建立标准化与系统化的检测方法,对提高生物检材进行年龄推断的准确性具有重要的意义。〈br〉   随着免疫学的长足发展,免疫系统结构和功能的增龄性变化规律以及在衰老过程中的作用也逐渐受到了研究者的普遍关注。作为中枢免疫器官,胸腺的增龄性功能退化是免疫系统在增龄和衰老过程中最突出的改变。定量分析外周血中特异的基因重排删除产物-信号结合T细胞受体重排删除环(Signal—joint Tcell recepter rearrangement excision circles,sjTRECs)水平是目前检测胸腺功能水平的可靠方法。研究证实,人外周血sjTRECs水平随增龄按一定的规律下降,有可能是年龄推断中的理想新指标。本课题以汉族健康无关个体为研究对象,拟建立一种简便、灵敏、特异、重复性好的实时荧光定量检测人外周血siTREC含量方法,用于构建正常人sjTREC水平的数据库,为客观评价胸腺输出功能提供实验依据,并初步评价外周血sjTREC含量在年龄推断中的潜在应用价值。〈br〉   本研究主要包括以下三个部分:〈br〉   第一部分:实时定量PCR检测sjTREC方法学建立〈br〉   目的:建立并优化实时荧光定量PCR技术检测人siTREC基因的方法。〈br〉   方法:设计并合成人sjTREC特异性引物与探针,使用TA克隆载体构建人sjTREC标准品,建立标准曲线。优化实时定量PCR反应体系及热循环参数,并对所建立的实时定量PCR方法的特异性、灵敏度以及重复性进行方法学评价。〈br〉   结果:基因组DNA经实时定量扩增后,可见清晰的扩增曲线,而阴性对照(双蒸水)未见明显扩增曲线。成功构建了人sjTREC的重组质粒作为标准品,建立了标准曲线,所得标准曲线斜率为-3.544,相关系数为0.999189,PCR反应动力学范围达6个log值(101-106),检测下限为lOcopies;稳定性检测中,三种浓度不同的标准品的批内变异和批间变异分别为0.09%、0.77%、1.68%和0.54%、1.88%、1.83%,均小于2%。〈br〉   结论:建立的实时荧光sjTREC定量方法对目的基因DNA拷贝数进行PCR反应线性范围广、敏感性高、稳定性好,可满足后续实验的要求。〈br〉   第二部分:实时荧光定量PCR检测正常人外周血sjTREC含量的研究〈br〉   目的:本研究利用第一部分试验中建立的sjTREC定量检测方法,对各年龄组正常人外周血sjTREC的含量进行分析,初步建立正常人TRECs水平的数据库。〈br〉   方法:使用试剂盒或酚/氯仿法提取收集的246例各年龄组健康人外周血基因组DNA样本,利用第一部分试验中建立的sjTREC定量检测方法,在ABI7500 PCR仪上实时扩增检测各年龄组正常人外周血样本中sjTREC片段,按照文献介绍的方法计算每毫升血液中所含的sjTREC拷贝数(sjTREC/ml),结果取对数值。sjTREC含量与年龄的关系作相关分析;各年龄组性别差异作t检验。〈br〉   结果:246例健康人外周血样本按照年龄分为14组(每隔5岁为1组)。sjTREC定量结果表明,外周血sjTREC水平随年龄增长而逐渐下降,与年龄呈显著负性相关(r=0.8323,p〈0.001),下降呈不均衡趋势:10~14组与15~19组的LogsjTREC/mL值差值最大,15~54岁之间各组检测值相差均较小,55~组以上与50~54组差值又相对较大。对0~14岁、15~54岁、55~岁三组进行方差分析,显示三组间有显著性差异(p〈0.001);均值多重比较结果(LSD检验)表明0~14岁与15~54岁组、15~54岁与55~岁组、0~14岁与55~岁组间的均值均有显著性差异(p〈0.0001);各年龄组男、女性外周血sjTREC水平均无显著性差异(p〉0.05)。定量结果受DNA抽提方法的影响。〈br〉   结论:使用实时荧光定量PCR方法,分析并提供了正常人各年龄组外周血中sjTREC含量,初步建立了正常人sjTREC水平的数据库,为客观评价胸腺输出功能提供了依据。由于使用单一的外参照,定量结果受DNA抽提效率的影响,因此有必要同时应用内标管家基因来标化结果并修正核酸抽提过程造成的目的基因拷贝数的变化,以提高检测的准确性。〈br〉   第三部分:人外周血sjTREC水平在年龄推断应用中的初步研究〈br〉   目的:探讨外周血sjTREC在年龄推断应用中的潜在价值。〈br〉   方法:选择人TATA盒结合蛋白(TATA box binding protein,TBP)基因作为内参基因,设计并合成人TBP特异性引物与探针,使用TA克隆载体构建人TBP标准品,建立标准曲线。使用建立的实时荧光定量PCR反应系统对提取的248例各年龄组健康人外周血样本DNA中sjTREC和内参基因TBP进行定量检测,计算sjTREC与TBP的平均Ct值,并计算sjTREC/TBP比值,结果取对数值,以log(sjTREC/TBP)表示。log(sjTREC/TBP)值与年龄作相关及回归分析;各年龄组性别差异作t检验。〈br〉   结果:248例外周血log(sjTREC/TBP)值含量随年龄增长逐渐下降(r=-0.8177);0~14岁内外周血siTREC水平随增龄下降的速度较慢,14岁(青春期)开始下降速度加快,与胸腺开始萎缩的时间一致,之后在相当长的一段时间内下降速度较缓,55岁开始又开始加快下降。对0~14岁、15~54岁及55~岁三组进行方差分析(LSD检验),显示有显著性差异(p〈0.0001);均值多重比较结果(LSD检验)表明0~14岁与15~54岁组、15~54岁与55~岁组、0~14岁与55~岁组间的均值均有显著性差异(p〈O.0001);各年龄组男、女性外周血sjTREC水平均无显著性差异(p〉O.05)。得到推断年龄的回归方程为:Y=-24.921x-39.932±10.47(Y:供体年龄,岁;X:log(sjTREC/TBP);±10.47:标准误);在所有被检测的248例样本中,162例(~66.0%)的年龄推测值与实际年龄误差在10岁之内,其准确率与文献报道的利用TRF长度推断个体年龄的相似(49.5%的年龄实际值与推测值之间误差小于10岁)。定量结果受DNA抽提效率的影响小。〈br〉   结论:使用内参照基因TBP标化结果能修正由于DNA抽提过程造成的目的基因拷贝数的变化,提高检测的准确性。人外周血中sjTREC含量与年龄存在明显相关性,为法医学在分子水平上推断个体年龄提供一个有潜在价值的新指标。
