Liegt ein Patient im Sterben, sind schwierige Entscheidungen zu treffen. Die ärztliche Indikation wird dabei von den Wertvorstellungen des Arztes sowie von seinen Annahmen über die Werte der Gesellschaft geprägt. Wenn der Patient entscheidungsunfähig geworden ist, ist jedoch sein Wille zumeist nicht eindeutig zu ermitteln. Patientenverfügungen können eine Hilfestellung geben, enthalten aber oft unspezifische Formulierungen, die keine konkrete Handlungsanweisung geben. Deshalb muss man davon ausgehen, dass es auch bei scheinbar eindeutiger »medizinischer Indikation« und Patientenverfügung im klinischen Alltag bei Entscheidungen am Lebensende zu ganz unterschiedlichen Ergebnissen kommt.
