Abstract:
Ziel der Studie Die Einführung des Pflegekomplexmaßnahmen-Score (PKMS) wird oft als Paradigmenwechsel im G-DRG-System bezeichnet. Ziel dieser Arbeit war
es, die durch das PKMS entstehende Anreizwirkung innerhalb des G-DRG-Systems zu analysieren. Methodik Mit Hilfe einer Sekundäranalyse mit Routine-Abrechnungsdaten wurden dazu alle im Jahr 2014 am Universitätsklinikum Freiburg behandelten
PKMS-Fälle untersucht. Ergebnisse Die durchschnittliche Mittlere Verweildauer der abgerechneten DRGs wurde von den PKMS-Patienten deutlich überschritten (16,5 Tage oder um 77,8
%, p 〈 0.001). Bei Annahme durchschnittlicher Krankenhauskosten reichen die im PKMS-System vorgesehenen Zusatzentgelte bei weitem nicht aus, um die Kosten
der Verweildauerüberschreitungen zu kompensieren. Schlussfolgerung Obwohl ein gewisser kompensatorischer Effekt des Zusatzentgeltes nicht ausgeschlossen werden kann, muss davon ausgegangen werden, dass die durch das PKMS-E-System gewährten Zusatzentgelte schlicht zu niedrig sind, um einen Einfluss auf die im G-DRG-System enthaltenen Anreize zur
Verweildauerreduktion zu entfalten.
