Abstract: Mental health strategies in old age necessitate inter-professional and inter-disciplinary approaches. Changes in mobility and comorbidity as well as the increasing demand of daily assistance cause challenges to the old age health care. Early disease detection in old age is essential as the length of disease and recurrence have a tremendous impact on health in old age. Studies have shown promising effects when applying stepped-care interventions, collaborative care, and assertive community treatment for old patients suffering mental diseases. Home-based mental health care programs combining primary care and old age psychiatric treatment have shown an improved identification of health problems, treatment and ongoing care of mental health problems. The effectiveness revealed for telemental health service may become an important approach to make specialized old age mental care available to a large number of underserved elderly with mental illness in Germany.
