    Abstract: Introduction : Osteochondral lesions of the talus are frequent and often associated with a trauma of the ankle. They pose a diagnostic and therapeutic problem. The clinical symptoms are not specific as for pain, stiffness, tumefaction, or other mechanical clinical signs. Although the natural history of these lesions is not fully understood, a surgical treatment is often necessary especially in case of chronic case or in acute case with free articular fragments. The autologous osteochondral grafts seem a promising treatment at a short follow-up. Materials and methods : Between 2016 and 2019, 27 patients have been treated by an osteochondral autogragft in osteochondral lesions of the talar head. The minimal follow-up is 1 year with a mean follow-up of 21 months (12–48 months). Results : The American Orthopedic Foot and Ankle Society mean scores rose from 45 to 83 points (72–96). One patient complained about the knee where the graft has been harvested, and another patient needs a removal of the osteosynthesis of the medial malleolus. Conclusions : According to our results, the autologous osteochondral graft is a satisfying and reproducible treatment of some osteochondral lesions of the talar head.
