Sebastian Kessler macht in seiner Analyse des Ungleichheitsdiskurses für den Zeitraum von 1975 bis 2009 auf zwei Entwicklungen aufmerksam: Soziale Ungleichheit in der Gesundheit ist ein wissenschaftliches Konzept, das ab 1975 einen neuen Blick auf die Wirklichkeit eröffnet. Es lassen sich nun gesellschaftliche Differenzen von Krankheit und Tod identifizieren und als Problem darstellen. Mit der politischen Aneignung dieses Konzeptes überträgt sich jedoch die Verantwortung für die Gesundheit von der gesamten Gesellschaft auf Institutionen wie die Krankenversicherungen oder auf den Einzelnen. Damit wird das Wissen über Ungleichheit zu einem Mittel der Verwaltung der Gesellschaft. Um der Kostenexplosion im Gesundheitswesen entgegenzuwirken findet durch die Konstruktion des Wissens über Ungleichheit eine Regulierung des Lebens in den Ungleichheitsverhältnissen statt.
