Die Gesundheitssysteme Deutschlands und der Schweiz zählen zu den teuersten der Welt, bringen aber nur mittelmäßige Gesundheitsergebnisse hervor, schöpfen mithin ihre Ressourcen und ihr Leistungspotential nur unzureichend aus. In beiden Ländern gilt die starke Abschottung von Versorgungssektoren - einhergehend mit Integrationsproblemen im Versorgungsprozess - und Versicherungsbereichen als wichtige Ursache für diese Ineffizienzen. Die fehlende Integration medizinischer, pflegerischer und sozialer Versorgung zeigt sich als unzureichende Kommunikation/ Koordination zwischen beteiligten Akteuren. Sie führt zur Segmentierung von Versorgungsprozessen und Brüchen im Behandlungsverlauf. Patient/-innen mit chronischen Erkrankungen mangelt es an langfristiger, kontinuierlicher und koordinierter Betreuung und Versorgung. Die Folgen sind suboptimale Behandlungsergebnisse und - damit einhergehend - ein Verlust an Lebensqualität für Patient/-innen, zum anderen eine Verschwendung von Ressourcen.

The health care systems of Germany and Switzerland belong to the most expensive in the world, yet they generate only average results in terms of health outcornes, thus exploiting their resources and potentials insufficiently. In both countries a strong compartmentalization of health care sectors - going along with integration problems regarding health care delivery - as weil as of insurance sectors is seen as major causes of these inefficiencies. The lack of integration between medical, nursing and social ca.re appear as insufficient communication and coordination between the actors involved This leads to a segmentation of health care processes and disruptions in patients' pathways. Longterm, sustainable and coordinated support and care are insufficient for patients with chronic diseases. Consequences are suboptimal health outcornes and - along with that - a loss of quality of life for patients, and a waste of resources on the other side.
